彩色多普勒诊断剖宫产切口瘢痕妊娠的临床价值分析

摘　　要：目的分析彩超对剖宫产切口瘢痕妊娠(CSP)的诊断价值。方法选择有临床出血症状的剖宫产史女性108例为研究对象,均经血β-人绒毛膜促性腺激素(β-h CG)检查确诊为早孕,进行经阴道和腹部彩色多普勒检查。均行手术治疗,以术中所见为金标准,计算彩色多普勒检查结果判定的CSP的灵敏度、特异度及准确度,计算符合率。结果 108例患者术中所见,CSP为39例(36.11%),彩色多普勒经阴道联合经腹部诊断CSP为37例。彩色多普勒经阴道和经腹部联合检查与手术所见的符合率为92.31%。彩色多普勒经阴道联合经腹部对出血性剖宫产史早孕患者CSP诊断的特异度为94.37%,灵敏度为94.59%,准确度为94.44%,明显高于单纯采用经阴道和经腹部检查的诊断结果。结论彩色多普勒经阴道联合经腹部检查是诊断剖宫产切口瘢痕妊娠并进行准确分型的简便、快速、准确方法,对因出血而就诊的剖宫产史早孕患者选择适合的治疗方案有较高的临床价值。Objective To analyze the diagnostic value of color Doppler ultrasonography for cesarean scar pregnancy（ CSP）.Methods A total of 108 women with hemorrhage symptom and cesarean section history were selected as study object,all the women were definitely diagnosed as early pregnancy by blood β-human chorionic gonadotropin（ β-h CG） detection,transvaginal color Doppler ultrasonography and transabdominal color Doppler ultrasonography were performed. All the women were treated by surgery,the surgical resutls were gold standards,the sensitivity,specificity,and accuracy of color Doppler ultrasonography in diagnosis of CSP were calculated,the conicidence rate was also calculated. Results Among 108 women,39 women were diagnosed as CSP,the incidence rate was 36. 11%; 37 women were diagnosed as CSP by transvaginal color Doppler ultrasonography combined with transabdominal color Doppler ultrasonography,the coincidence rate was 92. 31%. The specificity,sensitivity,and accuracy of transvaginal color Doppler ultrasonography combined with transabdominal color Doppler ultrasonography in diagnosis of CSP among the women with hemorrhage symptom and cesarean section history were 94. 37%,94. 59%,and 94. 44%,respectively,which were statistically significantly higher than those of single transvaginal color Doppler ultrasonography and single transabdominal color Doppler ultrasonography. Conclusion Transvaginal color Doppler ultrasonography combined with transabdominal color Doppler ultrasonography is simple,fast,and accurate in diagnosis of CSP and accurate classification,which has high clinical application value in choice of appropriate treatment plan among the women with hemorrhage symptom and cesarean section history.
