摘　　要：目的:探讨帕瑞昔布联合舒芬太尼对全麻下胸腔镜肺叶切除术围术期疼痛和炎性细胞因子的影响。方法:选取我院收治的行全麻下胸腔镜肺叶切除术患者54例,随机分为治疗组与对照组,每组27例。麻醉诱导前,治疗组患者静脉推注帕瑞昔布40 mg,对照组推注等量生理盐水,两组麻醉诱导与维持均用丙泊酚与舒芬太尼。记录两组患者的麻醉时间、手术时间、苏醒时间以及舒芬太尼用量;分别于术前、术毕以及术后6、12、24 h抽血,测定患者血液中IL-6、IL-8水平,并比较两组患者术后疼痛程度。结果:治疗组患者术后6、12、24h血液中的IL-6、IL-8水平明显低于对照组;疼痛评分也明显低于对照组,两组差异比均具有统计学意义(P<0.05)。结论:帕瑞昔布镇痛可抑制胸腔镜肺叶切除术后炎症反应,减少术中镇痛药用量,术后镇痛效果良好。Objective：To explore the effects of parecoxib combined with sufentanil on the perioperative pains and inflammatory cytokines of patients after video-assisted thoracosopic surgery（VATS）.Methods：Fifty-four patients und ergoing video-assisted thoracosopic surgery with general anesthesia in our hospital between August 2014 and August2015,were randomly assigned to one of two groups.The active treatment group（n=27） received parecoxib intravenou sly before anesthesia induction,while the control group（n=27） received equal saline solution.All patients were given propofol and sufentanil during anesthesia maintenance.Data from two groups were analyzed based on the duration of anesthesia,operation and recovering,sufentanil dosage,and blood IL-6 and IL-8 levels at different time points,as well as postoperative pain score.Results：Postoperative IL-6 and IL-8 levels at 6h,12 h and 24 h in the treatment group were significantly decreased compared with the control group（P〈0.05）.After surgery,the pain score was significantly lower in the treatment group than in the control group（P〈0.05）.Conclusion：Parecoxib treatment can reduce the dosage of analgesics,partially suppress postoperative inflammatory reaction and mitigate postoperative analgesia of patients with VATS.
