磁敏感加权成像对急性脑梗死动脉血栓的诊断价值

摘　　要：目的与TOF-MRA比较,探讨磁敏感加权成像(SWI)技术对急性脑梗死动脉血栓的诊断价值。方法58例急性脑梗死患者均行常规磁共振MRI和DWl、SWI、TOF-MRA检查。对SWI检测到的磁敏感血管征象(SWISVS)与MRA检测的脑动脉不同程度狭窄或闭塞频率通过相关统计学分析。结果 58例受检者,梗死区责任血管中,28例SWI-SVS阳性,25例MRA显示不同程度的狭窄或闭塞,其中25例SWI-SVS与MRA显示的闭塞或狭窄位置相匹配。3例SWI-SVS阳性,MRA为阴性。30例SWI-SVS与MRA均显示阴性。结论脑梗死急性期SWI与MRA显示脑动脉主要血管段血栓性闭塞有很好的一致性;SWI还可显示脑动脉血管远端细小分支血栓,与MRA相结合可提高脑动脉血管血栓性闭塞的检出率和准确性,为临床提供更全面可靠的影像信息。Objective：To evaluate the value of susceptibility weighted imaging （SW1） in diagnosing arterial thrombosis in acute cerebral infarction compared with TOF-MRA. Methods：Fifty-eight patients with acute cerebral infarction were per- formed with conventional MRI, DWI, SWI and TOF-MRA. The frequency of hypointense susceptibility vessel signs （SVS） on SWI （SWI-SVS） and vessel occlusion or stenosis on TOF-MRA were compared through the relevant statistical analysis. Results ： In 58 eases, 28 cases of SWI-SVS were positive, 25 cases of MRA were showed different degrees of stenosis or oc- clusion, and 25 eases of SWI-SVS matched with the occlusion or stenosis position of MRA. Three patients were positive for SWI-SVS, while MRA was negative; 30 cases of SWI-SVS and MRA showed negative. Conclusion： SWI and MRA in acute cerebral infarction show that cerebral artery thrombotic occlusion of major vascular segments have good consistency; SWI can also display cerebral artery distal branches of thrombus, and SWI combined with MRA can improve the detection rate and accuracy of cerebral artery occlusion to provide more comprehensive and reliable imaging information for clinical treat- meat.
