太极扣与磁性附着体在全口覆盖义齿中应用5年的效果比较

摘　　要：目的通过观察太极扣和磁性附着体的5年修复疗效,比较两者在全口覆盖义齿中的应用优劣差异。方法选取2009年10月至2011年9月在我院就诊需行覆盖全口义齿修复的牙列缺损患者36例,根据修复方式的不同分为太极扣组和磁性附着体组,太极扣组患者18例,采用根上太极扣修复,磁性附着体组患者18例,采用磁性附着体修复。两组患者修复后对其进行定期复查,通过记录5年的临床随访数据,对患者满意度、临床检查和X线片检查结果进行统计学分析。结果比较太极扣组和磁性附着体组患者满意度,两组在美观性、固位性、稳定性、咀嚼功能和舒适性方面均无统计学差异(P>0.05);比较太极扣组和磁性附着体组患者的临床检查和X线片检查结果,两组在基牙松动度、牙龈指数和牙槽骨吸收上的差异均无统计学意义(P>0.05);5年后随访,所有患者安装的义齿均在正常使用,其中有4例太极扣组患者换了1次阴性部件;有1例磁性附着体组的患者下颌尖牙磁铁处的人工牙在戴牙1.5年后脱落,重新修理后继续使用;两组均有超过60%的基牙发生不同程度的牙龈炎症。结论太极扣修复和磁性附着体修复在覆盖全口义齿修复中的5年疗效观察满意,两者各有所长,但是要注意适应症的选择。Objective：To observe the effect of 5 years of the treatment of Taijiquan and magnetic attachments, and to compare the difference of the application of the two methods in the complete overdenture. Methods ： From October 2009 to September 2011 in our hospital for overdenture dentition defect in patients with 36 cases, according to the different repair methods for Sterngnld group and magnetic attachment group, Sterngold group of 18 patients, the root of Sterngold repair, 18 cases of patients with magnetic attachment body, using magnetic attachment repair. Two groups of patients after the repair of regular review, by recording the clinical follow-up data for 5 years, the patient satisfaction, clinical examination and X- ray examination results were statistically analyzed. Results：The two groups were not statistically significant in appearance, retention, stability, comfort and masticatory function （P 〉 0.05） ; comparison of Sterngold group and magnetic attachment groups of patients with clinical examination and X-ray examination results, two groups in abutment loose difference degree, gingival index and alveolar bone absorption were not statistically significant （ P 〉 0.05 ） ; after 5 years of follow-up, all pa- tients were installed dentures in normal use, of which 4 cases of Sterngold patients had 1 negative components; artificial teeth in patients with 1 eases of mandibular canine magnet magnetic attachment group the tooth wear off in 1.5 years, con- tinue to use again after repair of abutment ; two groups were more than 60% gingivitis occurred in different degrees. Canclu- sion：Sterngold repair and restoration of magnetic attachment in complete overdanture restoration in 5 years to observe the curative effect of the two, each one has his good points but should pay attention to the indications, choice.
