黄芪桂枝五物汤加减外用防治卡培他滨相关性手足综合征23例临床观察

摘　　要：目的观察黄芪桂枝五物汤加减外用防治卡培他滨相关性手足综合征(HFS)的临床疗效。方法将46例接受卡培他滨化疗的乳腺癌、胃癌、大肠癌患者随机分为治疗组和对照组,各23例。对照组从化疗第1天开始口服甲钴胺片,0.5 mg/次,3次/d;治疗组从化疗第1天开始采用黄芪桂枝五物汤加减煎汤浸泡手足,2次/d,连泡21 d。2组均以21 d为1个疗程,6个疗程后比较2组患者的HFS发生率及严重程度。结果治疗组HFS总发生率为52.2%,对照组为73.9%,2组比较差异无统计学意义(P>0.05);治疗组Ⅱ度及以上HFS发生率为13.0%,对照组为47.8%,差异有统计学意义(P<0.05)。结论应用卡培他滨化疗时,预防性给予黄芪桂枝五物汤加减煎汤浸泡手足可以减轻卡培他滨致HFS的毒性,降低HFS发生的严重程度,改善肿瘤患者的生活质量,值得临床推广。Objective To observe the curative effect of modified Huangqi Guizhi Wuwu Tang for external use in preventing and treating capecitabine related hand foot syndrome （HFS）. Methods 46 malignant tumor patients,being under eapeeitahine chemotherapy,were randomly divided into treatment group and control group,23 cases for each. The patients in control group took mecobalamin tablets orally,0.5 mg each time,3 times a day ;the patients in treahnent group dipped their hands and feet into the Huangqi Guizhi Wuwu Tang,2 times a clay for 21 davs. 21 days were as one course for hoth groups. The incidences and severity of HFS in two groups were compared after six courses. Results The total incidences rates of HFS in the treatment group and control group were 52.2% and 73.9% respectively, the diffrence was not significant （P〉0.05）. However, the incidence rates of HFS on II-lewd and above H-level in treatment group and control group were 13.0% and 47.8% respectively, the diffefence was significant （P〈0. 05）. Conclusion Preventive treatment of infusion hy modified Huangqi Guizhi Wuwu Tang can reduce the toxicity and severity induced by capecitabine. The method can improve the life quality of tumor patients. It is worth popularizing and applying in clinic.
