苯磺酸氨氯地平对糖尿病合并高血压患者血压晨峰现象的影响

摘　　要：目的探讨苯磺酸氨氯地平在糖尿病合并高血压患者治疗中,对血压晨峰现象的控制作用。方法 108例2型糖尿病合并高血压患者,随机分为观察组与对照组,各54例。两组均采用二甲双胍联合沙格列汀控制血糖至正常范围。其中观察组采用苯磺酸氨氯地平降压治疗,对照组采用硝苯地平缓释片治疗。监测两组治疗后3~4、7~8、15~16、30~31 d是否出现血压晨峰现象,并进行组间比较。结果两组患者治疗期间,血糖值未有明显波动,均控制在正常范围内。观察组治疗后第3~4、7~8、15~16、30~31天的血压晨峰已控制例数分别为7、18、30、46例,对照组分别为6、15、19、25例。观察组在治疗后第3~4天与第7~8天血压晨峰已控制例数均高于对照组,但比较差异无统计学意义(χ~2=0.087、0.393,P>0.05);治疗后第15~16天与第30~31天,观察组患者的血压晨峰已控制例数均高于对照组,差异具有统计学意义(χ~2=4.520、18.130,P<0.05)。结论苯磺酸氨氯地平对2型糖尿病合并高血压患者的血压晨峰现象控制良好。Objective To investigate control effect by amlodipine besylate on morning blood pressure surge in the treatment of diabetes complicated with hypertension patients. Methods A total of 108 admitted diabetes complicated with hypertension patients were randomly divided into observation group and control group, with 54 cases in each group. Both groups received melbine combined with saxagliptin for blood glucose control. The observation group received amlodipine besylate for antihypertensive therapy, and the control group received nifedipine sustained release tablets for treatment. Occurrence of morning blood pressure surge was monitored in 3～4, 7～8, 15～16, 30～31 d after treatment for comparison between the two groups. Results Both groups showed normal blood glucose levels in standard range without obvious changes. The observation group had 7, 18, 30, 46 cases with controlled morning blood pressure surge in 3～4, 7～8, 15～16, 30～31 d after treatment respectively, and the control group had respectively 6, 15, 19, 25 cases. The observation group had both more cases with controlled morning blood pressure surge in 3～4 and 7～8 d after treatment than the control group, while their difference had no statistical significance（χ^2=0.087, 0.393, P〈0.05）. In 15～16 and 30～31 d after treatment, the observation group had more cases with controlled morning blood pressure surge than the control group, and their difference had statistical significance（χ～2=4.520, 18.130, P〈0.05）. Conclusion Amlodipine besylate shows excellent control effect on morning blood pressure surge in diabetes complicated with hypertension patients.
