糖尿病性黄斑水肿的光学相干断层扫描图像特征

摘　　要：目的通过光学相干断层扫描(OCT)来观察糖尿病性黄斑水肿(DME)患者的图像特征,并分析其变化与患者视力的关系。方法对2015年11月至2016年6月期间确诊的60例糖尿病性黄斑水肿患者,采用回顾性分析方法,分析患者黄斑中心凹厚度(CFT)及视网膜光感受器细胞层的内外节(IS/OS)连接的图像特征,并对患者的视力结合分析。结果患者黄斑中心凹厚度(R=-0.317,P=0.008)以及IS/OS连接完整性分级(χ~2=50.103,P<0.001)与患者视力呈负相关,同时DME患者黄斑中心凹厚度与IS/OS连接分级显著相关(R=0.419,P<0.001)。结论黄斑水肿厚度和IS/OS连接完整程度与患者视力密切相关,可在临床诊断及治疗中作为指导依据,指导临床上对患者的视力、预后的判断以及对患者进行随访,跟踪患者病情进展。Objective To observe the characteristics of image in patiente with diabetic macular edema （ DME） by optical coherence tomo-graphy （ OCT） , and to analyze the relationship between changes of OCT images and visual acuity. Methods A retrospective analysis was conduc-ted on the clinical data of 60 patients with DME during November 2015 to June 2016. The relationship of central foveal thickness （CFT） , charac-teristics of IS/OS junction and visual acuity of patients were analyzed. Results The CFT （ R = - 0. 317 , P = 0. 008 ） and the grade of IS/OS junction （）（2 = 50.103 , P 〈0.001） had negative correlation with visual acuity of patients with DME. The CFT and grade of IS/OS were signifi-cantly correlated （R=0.419,P 〈0.001）. Conclusion This study showed that CFT and grade of IS/ OS junction integrity are significantly cor-related with visual acuity of patients with DME. It can provide guidance to the judgement for visual acuity, prognosis and follow - up in the clinic.
