摘 要:目的:研究顺尔宁联合甲基泼尼松龙治疗对支原体肺炎患儿炎症反应、外周血淋巴细胞亚群含量的影响。方法:选择2014年6月~2016年10月期间在淮安市第二人民医院儿科就诊的74例重症支原体肺炎患儿作为研究对象并随机分为两组,干预组接受常规治疗联合顺尔宁、甲基泼尼松龙,对照组接受常规治疗。治疗前后采集血清并测定炎症细胞因子、T细胞亚群相应细胞因子的含量,采集外周血并测定T细胞亚群的含量。结果:治疗后3、7d时,两组患儿血清中单核细胞趋化蛋白(MCP)-1、降钙素原(PCT)、细胞间黏附分子(ICAM)-1、CXCL8、C反应蛋白(CRP)、γ干扰素(IFN-γ)、白介素(IL)-17的含量以及外周血中Th1、Th17的含量均显著低于治疗前,血清中IL-4、转化生长因子(TGF)-β的含量以及外周血中Th2、Treg细胞的含量均显著高于治疗前(P<0.05),且干预组患者血清中MCP-1、PCT、ICAM-1、CXCL8、CRP、IFN-γ、IL-17的含量以及外周血中Th1、Th17的含量均显著低于对照组,IL-4、TGF-β的含量以及外周血中Th2、Treg细胞的含量均显著高于对照组,差异均有统计学意义(P<0.05)。结论:顺尔宁联合甲基泼尼松龙治疗支原体肺炎能够抑制炎症反应并调节Th1/Th2、Th17/Treg细胞的平衡。Objective:To study the effect of Singulair combined with methylprednisolone treatment on inflammatory response and peripheral blood lymphocyte subset content in children with mycoplasma pneumonia.Methods:A total of 74 children with severe mycoplasma pneumonia who were treated in Huai'an Second People's Hospital between June 2014 and October2016were selected as the research subjects and randomly divided into two groups,the intervention group received routine treatment combined with Singulair and methylprednisolone,and control group received routine treatment.Before and after treatment,serum was collected to detect the contents of inflammatory cytokines and corresponding cytokines of T lymphocyte subsets,and peripheral blood was collected to determine the contents of T cell subsets.Results:Three days and seven days after treatment,serum MCP-1,PCT,ICAM-1,CXCL8,CRP,IFN-γand IL-17 contents as well as peripheral blood Th1 and Th17contents of both groups were significantly lower than those before treatment while serum IL-4and TGF-βcontents as well as peripheral blood Treg and Th2 contents were significantly higher than those before treatment(P〈0.05),and serum MCP-1,PCT,ICAM-1,CXCL8,CRP,IFN-γand IL-17 contents as well as peripheral blood Th1 and Th17contents of intervention group were significantly lower than those of control group while serum IL-4and TGF-βcontents as well as peripheral blood Treg and Th2 contents were significantly higher than those of control group(P〈0.05).Conclusions:Singulair combined with methylprednisolone treatment of mycoplasma pneumonia can inhibit the inflammatory response and regulate the balance of Th1/Th2 and Th17/Treg cells.
