摘  要:目的观察脑电熵指数及双频谱指数在小儿七氟烷麻醉深度监测中的效果。方法将300例择期行咽、腭扁桃体切除术的患儿随机分为3组,各100例。对照组(A组)不采用熵指数监测,试验1组(B_1组)状态熵(SE)目标值设定为55,试验2组(B_2组)SE目标值设定为45,比较3组麻醉深度监测效果。结果 B_1组在观察基础值(T_0)、气管插管前即刻(T_1)、气管插管后即刻(T_2)、扁桃体切除时(T_3)、腺样体刮除时(T_4)、拔管后5 min(T_5)时平均动脉压(MAP)水平与A组比较,差异均有统计意义(P<0.01),与B_2组比较,T_0,T_2,T_5时差异有统计学意义(P<0.05);B_1组在T_0,T_1,T_2,T_3,T_4,T_5时心率(HR)水平与A组比较,差异均有统计学意义(P<0.05),与B_2组比较,T_0,T_3,T_4,T_5时差异均有统计意义(P<0.05);B_1组在T_2,T_3,T_4时七氟烷呼气末浓度(EtSEV)水平与A组及B_2组比较,差异均有统计意义(P<0.01);B_1组呼吸恢复时间为(6.6±1.3)min,与A组及B_2组比较,差异均无统计学意义(P>0.05);B_1组苏醒睁眼、拔管时间分别为(6.8±1.2)min和(6.8±4.3)min,与A组及B_2组比较,差异均有统计学意义(P<0.05)。与其他两组比较,B_1组苏醒、拔管时间更短,七氟烷用量更少,血流动力学更稳定。结论脑电熵指数及双频谱指数用于小儿七氟烷麻醉深度监测均有一定效果,但脑电熵指数监测效果更佳。Objective To observe the effect of EEG entropy index and bispectral index in monitoring analgesic depth induced by sevoflurane anaesthesia in children. Methods Totally 300 children patients undergoing pharynx,palatine tonsillectomy were randomly divided into 3 groups,100 cases in each group. The control group( group A) did not use EEG entropy index monitoring,the target value of SE in the test group 1( group B_1) was set to 55,and the target value of SE in the test group 2( group B_2) was set to 45. The effects of anesthesia depth monitoring were compared among the 3 groups. Results The levels of MAP at immediately before intubation( T_1),immediately after intubation( T_2),tonsillecotmu time( T_3),adenoidectomy time( T_4) and 5 min after extubation( T_5) in group B_1 had significant differences with those in group A( P〈0. 05),and the levels of MAP at T_0,T_2,T_5 in group B_1 had significant differences with those in group B_2( P〈0. 05). The levels of heart rate( HR) at T_0,T_1,T_2,T_3,T_4,T_5 in group B_1 had significant differences with those in group A,and the levels of HR at T_0,T_3,T_4,T_5 in group B_1 had significant differences with those in group B_2( P〈0. 05).The levels of Et SEV at T_2,T_3,T_4 in the group B_1 were significantly different from group A and group B_2( P〈0. 01). The respiratory recovery time of group B_1 was( 6. 6 ± 1. 3) min,compared with the group A and the group B_2,the difference was not statistically significant( P〈0. 05). The recovery time and extubation time of group B_1were( 6. 8 ± 1. 2) min and( 6. 8 ± 4. 3) min,compared with the group A and the group B_2,the difference was statistically significant( P〈0. 05). Compared with the other two groups,the recovery time and extubation time of the group B_1 were shorter,the dosage of entropy and bispectral indexes in monitoring analgesic depth induced by sevoflurane anaesthesia was less,and the hemodynamics was more stable.
