摘 要:目的 分析脑卒中患者外周血中树突状细胞(DCs)的成熟度,及其共刺激分子CD80和CD86的表达水平.方法 选择在北京博爱医院神经康复科住院的脑卒中患者56例(脑梗死患者30例,脑出血患者26例)作为病例组,并依据美国国立卫生院神经功能缺损评分(NIHSS)标准评价卒中的严重程度,选择健康体检者20例作为健康对照组.采用流式细胞技术检测外周血中DCs的表面CD11c、HLA-DR以及共刺激分子CD80、CD86的表达水平.结果 脑出血和脑梗死患者外周血CD11c和HLA-DR双阳性DCs的比例明显降低(P〈0.001).脑出血患者成熟DCs中CD80和CD86阳性比例明显低于健康对照组(P=0.005,P=0.002),脑梗死患者CD80阳性和CD86阳性DCs比例也显著低于正常对照组(P=0.03,P=0.011),差异均具有统计学意义.此外根据NIHSS评分,NIHSS评分〉6组的脑梗死患者CD80和CD86阳性DCs的比例明显低于NIHSS组0-3组(P=0.04,P=0.018).结论 脑卒中患者外周血中DCs的成熟度下降,且DCs表面共刺激分子CD80和CD86表达均明显减少,脑出血和脑梗死患者外周血树突状细胞分化和成熟能力降低,可能与卒中后的免疫功能紊乱有关.Objective To investigate the maturity and expressions of co-stimulatory molecules(CD80 and CD86) of peripheral blood dendritic cells in patients with stroke.Methods We chose 56 patients(30 cases of cerebral infarction and 26 cases of cerebral hemorrhage),admitted to Beijing Boai Hospital from June to September 2014,as the subject investigated,and 20 healthy persons as normal controls.The severity of stroke was assessed with the National Institutes of Health Stroke Scale(NIHSS).Flow cytometry analysis was employed to detect the expression of CD11c,HLA-DR,CD80 and CD86 on DCs in the peripheral blood.Results The percentages of peripheral blood CD11+ and HLA-DR+ DCs in cerebral hemorrhage and cerebral infarction groups were significantly lower than that of the control group.Compared with normal controls,the expressions of CD80 and CD86 on DCs in cerebral hemorrhage group were significantly reduced(P=0.005,P=0.002),while the expressions of CD80 and CD86 in cerebral infarction group were also significantly decreased(P=0.03,P=0.011).The percentages of CD80+ and CD86+ DCs in cerebral hemorrhage patients in the NIHSS〉6 group were significantly lower than that in the NIHSS 0~3 group(P=0.04,P=0.018).Conclusion Patients with stroke has decreased levels of matured DCs and expressions of co-stimulatory molecules.The matured activities of peripheral blood DCs were greatly reduced in patients with cerebral hemorrhage and cerebral infarction,these may play an important role in the pathogenesis of immune disorders after stroke.
