摘 要:目的 探讨仪器法与光学显微镜法在尿液白细胞、红细胞分析中的临床应用价值.方法 随机选取498例本院门诊及住院患者中段尿液,分别用FUS-100全自动尿有形成分分析仪、H-800干化学分析仪及光学显微镜进行检测.结果 (1)白细胞的检出率由高到低依次为显微镜(28.51%)〉尿沉渣分析仪(24.90%)〉干化学分析仪(22.69%);红细胞的检出率由高到低依次为干化学分析仪(44.18%)〉显微镜(41.77%)〉尿沉渣分析仪(37.75%).(2)干化学分析仪与尿沉渣分析仪对白细胞和红细胞的检查结果差异均有统计学意义(P〈0.05).(3)以镜检法检出为标准,干化学分析仪对白细胞的敏感度为73.24%,特异度为97.47%,对红细胞的敏感度为86.54%,特异度为97.24%,;尿沉渣分析仪对白细胞的敏感度为84.50%,特异度为98.88%,对红细胞的敏感度为83.65%,特异度为91.03%.结论 干化学分析仪和尿沉渣分析仪均存在一定的假阳性和假阴性,联合应用3种检测方法,可减少误诊和漏诊率,为临床医生对疾病的诊断提供更加可靠的结果.Objective To discuss the clinical application value of the instrument method and the microscope in the analysis of white blood cells and red blood cells in urine.Methods 498 cases selected from our hospital were analyzed by FUS-100 urine sediment analyzer,H-800 dry chemical analyzer and microscope,respectively.Results (1)The proportion of white blood cells from high to low was microscope(28.51%) 〉 urine sediment analyzer(24.90%) 〉 dry chemistry analyzer(22.69%);the proportion of red blood cells from high to low was dry chemistry analyzer(44.18%) 〉 microscope(41.77%) 〉 urine sediment analyzer(37.75%).(2) The examination results of the white blood cells and red blood cells by dry chemistry analyzer and urine sediment analyzer showed statistically significance(P〈0.05).(3) Standard with microscopic examination,the white blood cells sensitivity was 73.24%,specificity was 97.47%,and the red blood cells sensitivity was 86.54%,specificity was 97.24% by dry chemical analyzer.On the other hand,the white blood cells sensitivity was 84.50%,specificity was 98.88%,and the red blood cells sensitivity was 83.65%,specificity was 91.03% by urinary sediment analyzer.Conclusion The dry chemistry analyzer and the urine sediment analyzer both have false positive and false negative.Combining three methods can reduce the ratio of misdiagnosis and missed diagnosis,and provide more reliable results for clinical diagnosis of disease.
