摘　　要：目的:探讨临床护理路径在社区高血压慢性病管理中的应用效果。方法:2016年1月选取中度危险的原发性高血压患者200例,将100例自愿加入者作为试验组,采用临床护理路径进行管理,其余100例作为对照组,采用社区常规服务方法进行管理,4个月后,观察两组高血压健康知识达标率、血压控制率和患者满意度。结果:试验组健康教育知识达标率从干预前的34%(34例)上升至干预后的92%(92例,P<0.01);对照组从干预前的32%(32例)上升至干预后的73%(73例,P<0.01)。干预后,两组血压控制水平均较干预前改善,试验组优于对照组(P<0.05)。试验组的满意度为92%,对照组为76%,组间差异有统计学意义(P<0.01)。结论:采用临床护理路径对社区高血压慢性病进行管理,能提高患者高血压健康知识达标率、血压控制率和患者满意度。Objective：To explore the application effect of clinical nursing pathway in the community chronic disease management of hypertension. Methods：In Jan. 2016, 200 cases of moderate risk essential hypertension were selected, among them 100 volunteers were randomly selected as an experimental group and managed with the clinical nursing pathway, and the remaining 100 cases were taken as a control group and managed with the community conventional service method. Hypertension health knowledge compliance rate, blood pressure control rate and satisfaction of the patients were observed in the two groups after 4 months. Results：The health education knowledge compliance rate in the experimental group increased from 34%（34 cases） before intervention to 92%（92 cases） after intervention（P0.01）, and the health education knowledge compliance rate in the control group increased from 32%（32 cases） before intervention to 73 %（73 cases） after intervention（P0.01）. After intervention, the levels of blood pressure control in both groups were better than those before intervention, and the experimental group was superior to the control group（P0.05）. The satisfaction rates were 92% in the experimental group and 76% in the control group, and the difference was statistically significant（P0.01）.Conclusion：Application of clinical nursing pathway for the community chronic disease management of hypertension can improve the compliance rate of the health knowledge, blood pressure control rate and the satisfaction degree of the patients with hypertension.
