椎管镜下腰椎间盘摘除术与改良小切口开窗椎间盘摘除术在腰椎间盘突出症的疗效对比

摘　　要：目的探究在治疗腰椎间盘突出症中采用椎管镜下腰椎间盘摘除术与改良小切口开窗椎间盘摘除术的疗效。方法将我院2014-03-2016-03诊疗的140例腰椎间盘突出症患者作为研究对象,均分为观察组与对照组,每组70例。对照组行椎管镜下腰椎间盘摘除术,观察组行改良小切口开窗椎间盘摘除手术,评价两组术后腰痛评分(JOA)及疼痛视觉模拟评分(VAS),对比两组患者手后并发症情况,以对比椎管镜下腰椎间盘摘除术与改良小切口开窗椎间盘摘除术在腰椎间盘突出症的疗效。结果两组患者术前JOA评分及VAS评分不存在显著差异(P>0.05);术后3周及6个月JOA评分及VAS评分均较术前明显改善,但差异不具有统计学意义(P>0.05);从术后并发症发生情况看,观察组术后并发症发生率(1.4%)低于对照组(2.8%),然对比不具有统计学意义(P>0.05)。结论改良小切口开窗椎间盘摘除术能取得与椎管镜下腰椎间盘摘除术一致治疗疗效,且改良小切口开窗椎间盘摘除术操作相对简便,可以作为治疗腰椎间盘突出症新的手术方式。Objective To investigate the efficacy of microendoscopic discectomy and modified small incision fenestration in the treatment of lumbar disc herniation. Methods 140 patients with lumnhar disc herniation in our hospital from March 2014 to March 2016 were selected as the research subjects, they were divided into observation group and eonlrol group, 70 cases in each group. The control group underwent microendoscopic hunbar discectomy, the observation group underwent small incision fenestration discecctomy surgery. The, l）OStoperative JOA and visual analogale scale （VAS） scores were evaluated in two grotlps. The tratmenl effect and complications werecompared between two groups. Results The JOA score and VAS score between two groups had no significant difference （P〉0.05）. 3 weeks and 6 months afler operation, JOA score and VAS score were significantly improved compared with preoperation, hut the difference was nnt statistically significant（P〉0.05）. The incidence rate of postoperative complications of the ohservation group （1.4%） was lower than 2.8% in the control group, the difference was not statistically significant （1）〉0.05）. Conclusion Improved small incision fenestration disceetomy and spinal canal can achieve favorable curative effect as endoscopilc lumbar discectomy, and improved small incision fenestration disce, ctomy operation is relatively simple, can be used as a new operation mode for the treatment of hunbar disc herniation.
