颈椎间盘置换与融合术后发生相邻节段退变的对比分析

摘　　要：目的综合分析颈椎间盘置换术(Cervical intervertebral disc replacement)后发生相邻节段退变情况。方法自2013-01-2016-07,选取本院收治的126例接受颈椎间盘置换术患者临床资料,按照不同的手术方法分为置换组与融合组各63例。置换组应用颈椎间盘置换术,融合组应用前路颈椎间盘切除融合术。对比两组患者术后相邻节段退变情况、手术前后JOA脊髓功能评分。结果 (1)术前,置换组和融合组患者JOA评分比较无统计学意义(P>0.05);术后,两组JOA评分显著高于手术前(P<0.05),但两组对比无统计学差异(P>0.05);(2)术后随访1年,置换组发生相邻节段退变率为9.52%(6/63),融合组为25.40%(16/63),置换组显著低于融合组(P<0.05)。结论颈椎间盘置换术能够有效治疗颈椎病患者,与此同时,能够在一定程度上降低相邻节段退变率。Objective To analyze the degeneration of adjacent segment after cervical intervertebral disc replacement. Methods From 2013-01-2016-07, 126 patients who underwent cervical disc arthroplasty were divided into two groups according to different operation methods： the replacement group and the fusion group, 63 cases in each group. Replacement group was treated with cervical disc replacement, and the fusion group was treated with anterior cervical discectomy. After surgery （after one year follow-up）, the adjacent segment degeneration, JOA spinal cord function score were compared between two groups. Results （~） Before operation, the JOA score between two groups had no significant difference （P〉0.05）. After operation, JOA score of the two groups was significantly higher than before operation （P〈0.05）, but the difference between two groups was not significant （P〉0.05）.（1）After one year follow-up, the degeneration rate of the adjacent segment in the replacement group was 9.52% （6/63）, which was significantly lower than 25.40% （16/63） in the fusion group （P〈0.05）. Conclusion Cervical disc replacement can effectively treat patients with cervical spondylosis, and at the same time, it can reduce the degeneration rate of adjacent segments to a certain extent.
