摘 要:目的研究绝经后妇女骨密度(Bone mineral density,BMD)与骨代谢标志物的关系。方法应用双能x线骨密度仪测量绝经期妇女腰椎BMD值,参照WHO的骨质疏松诊断标准,将109例绝经后妇女分为无骨质疏松(NOP)组,骨量减少组(OP1)组和骨质疏松(OP2)组,测定各组受试者BMD和电化学发光法检测骨代谢标志物包括血清骨钙素(OC)、I型原胶原N端肽(PINP)、I型胶原交联C-末端肽(β-CTX)、25羟维生素D(VitD—T)和甲状旁腺素(PTH)的水平。结果随着OP程度的加重,BMD值逐渐降低,而年龄逐渐增大;然而不同骨量组患者的OC、PINP、β-CTX和PTH等比较差异均无统计学意义(均P〉0.05)。结论绝经后骨质疏松患者血清骨代谢标志物水平未提示与骨密度存在联系;血清OC、PINP、β-CTX和PTH只反映绝经后妇女骨转换的高低,对绝经后骨质疏松的诊断无明显意义。Objective To investigate the correlation of bone turnover makers and bone mineral density (BMD) in postmenopausal women. Methods BMD value of the lumbar was measured by dual-energy X-ray absorption meter. 109 postmenopausal women were divided into non- osteoporosis group (NOP group), osteopenia group (OP1 group), and osteoporosis group (OP2 group) according to the BMD level. The serum bone turnover markers including osteocalcin (OC), N-terminal propeptide of type I procollagen (PINP),β collagen specific sequences (β-CTX), 25 hydroxy vitamin D (VitD-T), and parathyroid hormone (PTH) were detected by electrochemical luminescence method. Results With the aggravation of osteoporosis, the value of BMD gradually decreased, while the age increased. There were no statistically significant differences in serum levels of OC, PINP,β-CTX, and PTH among the three groups (P〉0.05). Conclusions No significant correlation has been found between the bone turnover makers and BMD in patients with postmenopausal osteoporosis. The serum levels of OC, PINP, βCTX, and PTH only reflect the level of bone remodeling in postmenopausal women, while have limited value for diagnosis of osteoporosis.
