滋阴养血清热汤治疗老年类风湿性关节炎临床研究

摘　　要：目的:探讨滋阴养血清热汤治疗老年类风湿性关节炎的临床疗效。方法:将72例老年类风湿性关节炎患者随机分成对照组和治疗组,每组36例。两组患者均接受一般治疗。对照组加用甲氨蝶呤片及叶酸治疗,治疗组加用滋阴养血清热汤治疗,两组患者均连续治疗3个月。检测血清免疫球蛋白、血清炎症因子水平及实验室指标,观察主要临床表现及不良反应,比较临床疗效。结果:与治疗前比较,两组患者血清免疫球蛋白水平、血清炎症因子水平及实验室指标水平均明显降低,晨僵时间、休息痛、关节压痛指数以及肿胀指数明显降低,差异有统计学意义(P<0.05);与对照组治疗后比较,治疗组血清免疫球蛋白、白细胞介素-1、肿瘤坏死因子及实验室指标水平均显著降低,晨僵时间更短,两组比较,差异有统计学意义(P<0.05)。治疗后治疗组有效率为88.89%,与对照组75.00%比较,差异有统计学意义(P<0.05)。两组患者不良反应率比较,无明显差异(P>0.05)。结论:滋阴养血清热汤治疗老年类风湿性关节炎具有显著疗效且安全可靠。Objective：To investigate the efficacy and safety of Ziyin Yangxue Qingre Decoction in treatment of senile Rheumatoid arthritis.Methods：72 cases of elderly patients with Rheumatoid arthritis were randomly divided into two groups,with 36 cases each group.Both groups received general treatment,and in addition patients in control group were treated with Methotrexate Tablet and Folic acid and the treatment group received Ziyin Yangxue Qingre Decoction in treatment.All patients were treated for 3 months continuously.The Serum immunoglobulin,serum inflammatory factor levels and laboratory indexes as well as the clinical performance and the adverse reactions of both groups were observed and the clinical efficacyies were compared before and after treatment.Results：Compared with those before treatment,the levels of serum immunoglobulin,serum inflammatory cytokine and laboratory indexes of the two groups significantly decreased and the indexes of morning stiffness time,rest pain,joint tenderness and swelling were significantly lower than those before treatment（P〈0.05）.Compared with those of the control group,the levels of serum immunoglobulin,interleukin-1,tumor necrosis factor and laboratory indexes of the treatment group were significantly lower（P〈0.05）.After treatment,the effective rate of the treatment group was 88.89%,which was significantly higher than that（75.00%）of the control group（P〈0.05）.There was no significant difference in adverse reaction rate between the two groups（P〈0.05）.Conclusion：Ziyin Yangxue Qingre Decoction is effective and safe in treatment of senile Rheumatoid arthritis.
