α-糜蛋白酶囊内冲洗联合聚桂醇囊内注射在腘窝囊肿硬化治疗中的应用

摘　　要：目的:探讨α-糜蛋白酶囊内冲洗联合聚桂醇囊内注射在腘窝囊肿硬化治疗中的应用。方法:选取2014年8月-2015年2月笔者所在医院收治的25例腘窝囊肿患者,男10例,女15例,年龄40~65岁,左侧13个,右侧12个,共25个囊肿,囊肿大小20 mm×31 mm×20 mm^60 mm×50 mm×45 mm。采用套管针经皮穿刺抽吸囊液,α-糜蛋白酶生理盐水溶液冲洗囊腔,聚桂醇囊内注射治疗,术后2周根据临床查体及超声检查判定疗效。结果:1次治愈17例(68.00%),2次治愈3例(12.00%),3次治愈2例(8.00%),无效3例(12.00%)。所有接受治疗的患者均未出现局部红肿疼痛、皮肤坏死、皮下硬结、遗留瘢痕及影响关节功能等并发症。结论:α-糜蛋白酶囊内冲洗联合聚桂醇囊内注射硬化治疗腘窝囊肿是一种微创且安全有效的方法,值得在临床中推广。Objective：To investigate the clinical effect of Alpha-chymotrypsin intracystic rinsing combined with Lauromacrogol intracystic injection in sclerotherapy of popliteal cyst.Method：From August 2014 to February 2015,25 patients with popliteal fossa cyst were selected,10 male,15 female,40-65 years old,13 with a left cyst,12 with a right cyst,25 patients totally with 25 cysts.The cysts ranged from 20 mm×31 mm×20 mm to 60 mm×50 mm ×45 mm in size） were treated with Alpha-chymotrypsin intracystic rinsing combined with Lauromacrogol intracystic injection.Result：After treatment,17（85.00%）,3（12.00%） and 2（8.00%） cysts disappeared after 1,2 and 3 times of sclerotherapy respectively,and 3（12.0%） cysts was ineffective.There were no complications such as red swelling,pain,skin necrosis,subcutaneous induration,scar formation and joint dysfunction in patients under sclerotherapy.Conclusion：Alphachymotrypsin intracystic rinsing combined with Lauromacrogol intracystic injection in sclerotherapy of popliteal cyst is not only a minimally invasive measure but also a safe and effective one.Its application is worth promoting in clinical treatment.
