摘 要:目的探讨桥接组合式固定系统治疗股骨髁间粉碎性骨折的临床效果。方法回顾性分析解放军59中心医院骨关节外科2014年5月-2016年9月使用桥接组合式内固定系统治疗的10例股骨髁间骨折患者,其中男性7例,女性3例;年龄19~60岁,平均45岁。致伤原因:道路交通伤6例,高处跌伤4例。骨折Müller AO分型为:33-C2 6例,33-C3 4例。评估手术时间、出血量及患肢功能恢复情况。结果 10例获随访12~30个月(平均19个月),手术时间1~2h(平均1.6h),出血量200~400mL(平均250mL),随访期间手术切口愈合良好,内固定无松动及断裂等并发症。术后Sanders肢体评价:优7例,良2例,可1例,优良率90%。结论使用桥接组合式固定系统治疗股骨髁间粉碎性骨折符合生物学固定原则,术中操作简单,临床效果满意,是治疗股骨髁间粉碎性骨折的一种新的内固定装置。Objective To discuss the clinical effect of bridging combined fixation system for the treatment of comminuted femoral condylar fracture. Methods Retrospective analysis was carried out on 10 cases of femoral intercondylar fracture from May 2014 to Sep. 2016. They were treated by bridging combined with fixation system. There were 7 males and 3 females aged 19 - 60 years ( average, 45 years ). Cause of injury included 6 cases of road traffic injuries and 4 cases of high falling. According to AO classification, 6 cases were 33C2 and 4 cases were 33C3. The time of operation, the amount of bleeding and the recovery of function of the affected limb were evalua- ted. Results Ten patients were followed up for 12 -30 months { mean, 19). The operation time ranged from 1 to 2 h (average 1.6h) , the amount of bleeding was 200 -400mL (average, 250mL). During the follow -up period, the surgical incision healed well, without internal fixation loosening, breakage or other complications. According to Sanders limbs evaluation after operation, there were excellent in 7 cases, good in 2 cases, and fair in 1 case, and the excellent and good rate was 90%. Conclusion The use of bridging combined with fixation system in the treat- ment of comminuted femoral intercondylar fracture conforms the principle of biological fixation with simple operation and satisfactory clinical effect. It is a new effective internal fixation device for the treatment of femoral intercondylar comminuted fracture.
