解剖型锁定钢板治疗尺骨鹰嘴粉碎性骨折的疗效分析

摘　　要：目的探讨解剖型锁定钢板治疗直接暴力机制所致的尺骨鹰嘴粉碎性骨折的方法和临床疗效。方法自2011年1月至2015年9月采用解剖型锁定钢板治疗直接暴力机制所致的尺骨鹰嘴粉碎性骨折21例，其中MayoⅡB型17例，ⅢB型4例，同时对并发的肘关节周围软组织损伤进行了相应处理。结果术后19例获得随访，2例失访，随访时间12～24个月，平均16个月，骨折均愈合，无伤口感染等并发症，肘关节功能按Bmberg-Morrey评分：优11例，良6例，可2例。结论解剖型锁定钢板内固定是治疗直接暴力机制所致的尺骨鹰嘴粉碎性骨折的有效方法，优点是可以提供早期功能锻炼的固定强度，但前提是对肘部软组织进行良好的综合处理。Objective To investigate the method and clinical effect of anatomic locking plate fixation in the treatment of comminuted olecranon fractures caused by direct violence. Methods From January 2011 to September 2015, 21 cases of comminuted olecranon fractures caused by direct violence were treated with anatomic locking plate fixation. According to Mayo classification, 17 cases were type Ⅱ B, 4 cases were type Ⅲ B. The soft tissue injuries around elbow joint were also treated. Results Postoperatively 19 patients were follow-up for 12 to 24 months with an average period of 16 months. Two cases lost follow-up. All the fractures were healed without wound infection or other complications. The elbow joint function was evaluated by Broberg-Morrey grading system. The results were rated as excellent in 11 cases, good in 6 cases, and fair in 2 cases. Conclusion Anatomic locking plate fixation is an effective method for treatment of comminuted olecranon fractures caused by direct violence, which provides the stability of early functional training. The treatment of soft tissue injuries around elbow joint is also very essential.
