摘　　要：目的:分析卵巢不含脂肪成熟性畸胎瘤MRI特征,旨在提高术前诊断正确率。方法:回顾性分析15例卵巢不含脂肪成熟性畸胎瘤影像特征,所有病例均经手术病理证实并行MRI常规序列平扫及增强扫描。主要观察肿瘤的部位、形态、大小、信号、强化特点。结果:15例表现均为囊性肿块,大囊6例,中囊6例,小囊3例,T_1WI大部分呈等、低信号,T_2WI呈等、高信号,DWI上大部分呈高信号,部分呈低信号,增强扫描肿瘤呈囊壁、分隔轻度强化。结论:卵巢不含脂肪成熟性囊性畸胎瘤有一定的特点:20~40岁育龄女性好发,单侧卵巢,多为大中囊,单房或多房,囊内两种以上成分,瘤体边界清楚,边缘无分叶,增强囊壁及分隔轻度强化,囊内可见轻度或无强化。当临床遇到有类似MRI表现的卵巢肿瘤时,术前鉴别诊断应考虑到本病。Objective：To improve the MRI diagnostic accuracy by investigating the imaging characters of the ovarian mature teratoma without fats.Methods：The imaging characters of 15 cases the ovarian mature teratoma without fats by surgery and pathology were retrospectively analyzed.Routine MRI and contrast-enhanced were performed in all patients.The tumour characters including locations,form,size,signal,enhancment and edema were observed.Results：cystic masses uniform showed in 15 cases,6 macrocystic cases,6 middlecystic cases,3 microcystic cases,they were mostly showed equal and low signal in T_1WI,equal and high signal in T_2WI,mostly showed high signal in DWI,partly showed low signal in DWI.light enhancements for cystic wall and septum in tumours.Conclusion：Some characters of the ovarian mature cystic teratoma without Fats can been observed,such as ofen occurred in women at 20~40 reproductive age,One-side ovary,macrocystic and middlecystic type in many cases,more than two components within the cyst,unilocular or multilocular,the clear margin,not edge divided,light enhancement for cystic wall and septum,light or no enhancements for components within the cyst.If facing to ovarian tumours with these characters in clinical,intraparenchymal ependymoma should be considered
