摘  要:目的:探讨2型糖尿病视网膜病变患者血清25(OH)D3、分形素趋化因子(FKN)、细胞间黏附因子-1(ICAM-1)水平变化,并分析25(OH)D3与FKN、ICAM-1的相关性。方法:选取2型糖尿病视网膜病变患者124例,根据其糖尿病视网膜病变严重程度分为非增殖期糖尿病视网膜病变(NPDR)组62例,增殖期糖尿病视网膜病变(PDR)组62例,同时选取2型糖尿病患者62例作为对照组,采用全自动生化检测仪生化指标,采用酶联免疫吸附实验检测25(OH)D3、FKN、ICAM-1的水平,分析2型糖尿病视网膜病变患者血清25(OH)D3水平与FKN、ICAM-1及相关临床指标的相关性。结果:与对照组比较,NPDR组患者血清25(OH)D3浓度明显降低,而ICAM-1、FKN浓度均明显升高,差异有统计学意义(均P<0.05);而与NPDR组比较,PDR组患者血清25(OH)D3浓度明显降低,ICAM-1、FKN浓度均明显升高,差异有统计学意义(均P<0.05)。进一步Pearson直线相关分析结果显示:血清25(OH)D3水平与收缩压、总胆固醇、低密度脂蛋白、糖化血红蛋白、糖化白蛋白、血糖、血肌酐、血尿酸、糖尿病病程、ICAM-1、FKN均呈明显负相关(r分别为-0.514、-0.437、-0.608、-0.703、-0.564、-0.417、-0.712、-0.325、-0.621、-0.774、-0.801,均P<0.01或P<0.05)。结论:随着糖尿病视网膜病变的逐渐加重,血清25(OH)D3水平逐渐降低,ICAM-1、FKN水平逐渐升高,血清25(OH)D3水平与ICAM-1、FKN呈明显负相关,提示25(OH)D3、ICAM-1、FKN共同参与了糖尿病视网膜病变的发生、发展。Objective: To investigate the levels of serum 25 Hydroxy Vitamin 12)3, fractalkine and intercellular adhesion molecule in the patients with type 2 diabetic retinopathy,and analysis the relationships between 25 Hydroxy Vitamin D3 and fractalkine, intercellular adhesion molecule. Methods: There were 124 patients with type 2 diabetic retinopathy (there were divided into background diabetic retinopathy group and proliferative diabetic retinopathy group based on the fundus fluorescein angiography) and 62 patients with 2 diabetic but not diabetic retinopathy(the control group) se- lected for the study. The levels of serum 25 Hydroxy Vitamin I)3, fractalkine and intercellular adhesion molecule were detected with Enzyme linked immunosorbent assay. The biochemical indicators were detected by automatic biochemical detector. Results:Compared with the control group,the levels of serum 25 Hydroxy Vitamin D3 in the background dia- betic retinopathy group and the proliferative diabetic retinopathy group were significantly decreased(both P〈0. 05); the levels of serum fractalkine, intercellular adhesion molecule were significantly increased(both P〈0. 05). Compared with the background diabetic retinopathy group, the levels of serum 25 Hydroxy Vitamin D3 in the proliferative diabetic retinopathy group was significantly decreased(both P〈0. 05)the levels of serum fractalkine, intercellular adhesion molecule were significantly increased(both P〈0. 05). Correlation analysis revealed that the levels of serum 25 Hydroxy Vitamin D3 in the 2 diabetic retinopathy group were negative correlated with systolic blood pressure,total cholesterol , low density l ipoprotein, glycosylated hemoglobin, glycated albumin, blood glucose, serum creatinine, blood uric acid, du- ration of diabetes, fractalkine and intercellular adhesion molecule(r= 0. 682,0. 599, both P〈0. 01). The levels of serum VEGF in the experiment group were negative correlation with SV and DV(r= -- 0. 514, -- 0. 437, -- 0. 608, -- 0. 7
