严重创伤患者主要照顾者创伤后成长现状及影响因素研究

摘　　要：目的了解严重创伤患者主要照顾者的创伤后成长水平并探讨其相关影响因素。方法采用患者及照顾者一般情况调查表、一般自我效能感量表(general self-efficacy scale,GSS)、社会支持评定量表(scocical support rating scale,SSRS)和创伤后成长评定量表(posttraumatic growth inventory,PTGI)对某三级甲等综合医院急诊就医的177例严重创伤患者的主要照顾者进行问卷调查。结果严重创伤患者主要照顾者的PTGI得分为(61.06±17.95)分,除自我转变维度以外,总分及其他4个维度得分均高于中间值;照顾者的性别、年龄、文化程度、与患者的关系、社会支持和自我效能感是创伤后成长的影响因素,共可解释方差变异量的42.7%。结论严重创伤患者主要照顾者在创伤发生后存在一定程度的创伤后成长,临床医护人员应全面评估照顾者的个性特征,提升照顾者的自我效能感,调动社会支持力量,以改善照顾者创伤后成长和心理健康水平。Objectives To investigate the growth levels of the main carers of the patients with severe trauma and the related influencing factors.Methods The self-designed general condition questionnaire for patients and carers,the general self-efficacy scale（GSS）,the social support revalued scale（SSRS）and the posttraumatic growth inventory（PTGI）were adopted to investigate the main carers of 177 patients with severe trauma receiving emergency treatment in a certain grade three class A comprehensive hospital.Results The average PTGI score of the main carers of the patients with severe trauma was（61.06±17.95）.Except the self-transformation dimension,the total score and that of the other dimensions were above the median.The sex,age,education degree,relationship with the patient,social support and self-efficacy of the carers were the influencing factors;they can explain 42.7% of the amount of variability altogether.Conclusions A certain extent of posttraumatic growth occurs to the main carers of the patients with severe trauma.The clinical workers should evaluate the personality of the carers in a comprehensively way to enhance the self-efficacy and mobilize the social support,so as to improve their posttraumatic growth and phychological health level.
