摘 要:目的探讨贫血患者应用血液检验对疾病鉴别诊断的效果。方法本研究选取2015年5月—2016年10月我院收治的48例地中海贫血患者和44例缺铁性贫血患者为研究对象,并选取同期的35例健康体检者为对照组。比较三组对象的血液检查指标及符合率等。结果地中海组和缺铁性组患者的平均红细胞体积(MCV)、平均血红蛋白含量(MCH)、红细胞血红蛋白含量(Hb)水平明显低于其对照组,数据比差异有统计学意义(P<0.05);地中海组血红细胞计数(RBC)、平均红细胞容积比值(RBC/MCV)、平均血红蛋白含量(MCH)及Hb水平均明显高于缺铁性组,数据比较差异有统计学意义(P<0.05);但平均红细胞体积(MCV)、红细胞体积分布宽度(RDW)水平明显低于缺铁性组,数据比较差异有统计学意义(P<0.05)。地中海组贫血患者血液检验的敏感度(95.83%)、特异性(64.58%)及符合率(79.17%)与缺铁性组的敏感度(93.18%)、特异性(65.91%)与符合率(81.82%)比较,差异无统计学意义(P>0.05)。结论血液检验对贫血患者诊断的符合率较高,且具有较高的灵敏度和特异性。Objective To investigate the effect of blood test on differential diagnosis of anemia. Methods This study selected 48 cases of thalassemia patients and 44 cases of iron deficiency anemia in our hospital from May 2015 to October 2016 as the research object, 35 healthy subjects were selected as control group. The blood test indexes and coincidence rate of the three groups were compared. Results The mean corpuscular volume(MCV), mean hemoglobin content(MCH) and erythrocyte hemoglobin content(Hb) in the Mediterranean group and the iron deficiency group were significantly lower than those in the control group, the difference of data ratio was statistically significant(P〈0.05); the red blood cell count(RBC), mean corpuscular volume ratio(RBC/MCV), mean hemoglobin content(MCH) and Hb levels in the Mediterranean group were significantly higher than those in the iron deficiency group, the difference between the two groups was statistically significant(P〈0.05); But the average red cell volume(MCV) and the volume distribution width(RDW) of erythrocytes were significantly lower than those of iron deficiency group, the difference between the two groups was statistically significant(P〈0.05). Mediterranean anemia group blood test sensitivity(95.83%) and specificity(64.58%) and accuracy(79.17%) and iron deficiency group sensitivity(93.18%) and specificity(65.91%) and accuracy(81.82%) comparison, the difference was not statistically significant(P〉0.05). Conclusion Blood test has higher coincidence rate with anemia, and has higher sensitivity and specificity.
