摘　　要：目的探究股骨颈骨折老年患者硬膜外麻醉穿刺应用不同镇痛方式的效果。方法选取2015年2月—2017年2月本院收治的69例股骨颈骨折老年患者为研究对象。将患者分为髂筋膜腔隙阻滞组、舒芬太尼组、对照组(不使用镇痛方式)。观察对比相关指标。结果髂筋膜腔隙阻滞组、舒芬太尼组在T1、T2的VAS评分均较对照组低(P<0.05)。体位改变时VAS评分:髂筋膜腔隙阻滞组<舒芬太尼组<对照组(P<0.05);患者舒适度:髂筋膜腔隙阻滞组>舒芬太尼组>对照组(P<0.05);呼吸抑制:髂筋膜腔隙阻滞组少于舒芬太尼组(P<0.05)。结论髂筋膜腔隙阻滞镇痛应用于硬膜外麻醉穿刺的股骨颈骨折老年患者有助于减轻体位改变时的疼痛,提高舒适度,使患者更易于接受椎管内麻醉操作,其镇痛效果确切。Objective To explore the effect of different analgesia on epidural anesthesia puncture in elderly patients with femoral neck fractures. Methods 69 cases of femoral neck fractures in elderly patients from February 2015 to February 2017 in our hospital were taken as the research object. The patients were divided into fascia iliaca compartment block group, sufentanil gr6up and control group （no analgesia）, with The related indices were compared. Results The score of VAS in the fascia iliaca compartment block group, sufentanil group in T~, T2 were significantly lower than the control group （P 〈 0.05）. With regard to posture changing VAS score, fascia iliaca compartment block group 〈 sufentanil group control group （P 〈 0.05）; with regard to patients＇comfort, fascia iliaca compartment block group 〉 sufentanil group 〉 control group （P 〈 0.05）; with regard to respiratory inhibition, fascia iliaca compartment block group was less than sufentanil group （P 〈 0.05）. Conclusion The application of fascia iliaca compartment block analgesia in epidural puncture on elderly patients with femoral neck fracture can relieve the pain when patients are changing position, improve the confortability, make patients more easily accept the spinal anesthesia operation, it has good analgesic effect.
