输注红细胞对骨髓增生异常综合征患者T细胞亚群及Treg的影响

摘　　要：目的 探讨输注红细胞对骨髓增生异常综合征（MDS）患者T细胞亚群及Treg细胞的影响.方法 收集MDS患者30例,分析输血前后外周血CD3＋CD4＋、CD3＋CD8＋及CD4＋CD25＋细胞比例.结果 MDS患者输血后1周CD3＋CD4＋T细胞上调,2周后水平下降,与输血前相当;CD3＋CD8＋T细胞则在输血后1周较输血前未见明显变化,但2周后上调并高于输血前水平;输血1周后CD4/CD8比例上调,但2周后水平显著降低,低于输血前水平;Treg细胞比例则在输血后逐渐上调,2周后水平高于1周及输血前水平.结论 输注红细胞可上调MDS患者外周血免疫抑制性细胞CD8＋T细胞和Treg比例,CD4/CD8比例下降,随时间推移输注红细胞可对机体产生免疫抑制功能.Objective To investigate the impact of RBC transfusion on T lymphocyte subsets and Treg in patients with myelodysplastic syndrome （MDS）.Methods Thirty patients with MDS were selected, CD3＋CD4＋, CD3＋CD8＋and CD4＋CD25＋T lymphocyte subpopulations before and after RBC transfusion were analyzed using flow cytometry.Results The frequencies of CD3＋CD4＋T cells was upregulated one week after transfusion, and decreased to the same level as previously after two weeks.And CD3＋CD8＋T cells was only upregulated two weeks after transfusion.The rate of CD4/CD8 was upregulated one week after transfusion, and decreased to the lower level than before two weeks after transfusion.However, Treg cells gradually increased after transfusion, the level two weeks after transfusion was higher than one week after transfusion and before transfusion.Conclusions RBC transfusion can upregulate the immunosuppressive cells Treg and CD8＋T in MDS patients, and can decrease CD4 / CD8 ratio.And over time, transfusion of red blood cells can restrain the body＇＇s immune function.
