依托咪酯联合地佐辛与异丙酚联合地佐辛在无痛人流麻醉过程中的临床有效性

摘　　要：目的 对比研究依托咪酯联合地佐辛与异丙酚联合地佐辛在无痛人流麻醉过程中的临床有效性.方法 收集2014年12月至2016年3月实施无痛人流手术的161例孕妇为研究对象,随机分为两组,A组82例患者给予依托咪酯联合地佐辛进行麻醉,B组79例患者给予异丙酚联合地佐辛进行麻醉.对比两组患者的各项生命体征指标、时间指标、VAS评分以及不良反应情况.结果 麻醉前2 min两组心率（HR）、血氧饱和度（SpO2）、收缩压（SBP）以及舒张压（DBP）生命体征间比较差异未见统计学意义（P〉0.05）;手术开始后2 min,A组各项生命体征指标与B组相比差异有统计学意义（P〈 0.05）;A组在苏醒后2 min的各项生命体征指标与麻醉前2 min比较差异未见统计学意义（P〉0.05）;而B组在苏醒后2 min的SBP和DBP与麻醉前2 min比较差异有统计学意义（P＆lt;0.05）;A组VAS评分、恢复意识时间以及出院时间都明显低于B组（P＆lt;0.05）;A组睁眼时间和恢复行走时间与B组比较差异未见统计学意义（P〉0.05）;A组中共35例（42.68%）患者发生了不良反应,显著低于B组的47例（59.49%）,χ2=4.5499, P=0.0329.结论 对于无痛人流患者,给予依托咪酯联合地佐辛进行麻醉的效果好,VAS评分低、恢复意识时间和出院时间短、不良反应率低,对生命体征影响小.Objective To compare the clinical effects of etomidate combined with dezocine and propofol combined with dezocine on the process of anesthesia during painless abortion.Methods From December 2014 to March 2016, 161 cases of pregnant women were treated and implemented painless operation in hospital.They were randomly divided into two groups, 82 patients in group A were given etomidate combined with dezocine anesthesia, 79 patients in the group B were given propofol combined with dezocine anesthesia.The vital signs, time index, VAS scores and adverse reaction conditions in the two groups were tested and compared.Results Two min before anesthesia, the heart rate （HR）, oxygen saturation （SpO2）, systolic blood pressure （SBP） and diastolic blood pressure （DBP） of two groups had no significant differences （P〉0.05）;2 min after operation, the vital signs of group A compared with the group B had obvious differences （P〈 0.05）;the vital signs of patients in group A 2 min after awake compared with 2 min before anesthesia, had no significant difference （P〉0.05）;the SBP and DBP in group B 2 min after awake compared with 2 min before anesthesia, there was significant difference （P〈 0.05）;the VAS scores, the time of consciousness recovery and discharge time in group A were significantly lower than in group B （P〈 0.05）;the eye opening and resuming time in group A compared with the group B with no difference （P〉0.05）;35 cases （42.68%） adverse reactions occurred in patients with group A, lower than that in group B of 47 cases （59.49%）（χ2=4.5499, P=0.0329）.Conclusions Etomidate combined with dezocine anesthetic has good effect on patients with painless induced abortion, has lower VAS score, shorter time of consciousness recovery and discharge, lower adverse reaction rate, little influence on vital signs.
