子宫横向环形压迫缝合术治疗产后出血59例临床分析

摘　　要：目的结合临床实践经验,开展子宫横向环形压迫缝合术治疗和预防剖宫产术中产后出血的临床分析。方法选取泰安市中心医院2016年5月—2016年9月期间59例产后出血患者为研究对象,采用回顾性研究,探讨子宫横向环形压迫缝合术治疗和预防剖宫产术中产后出血的临床疗效。结果 (1)疗效:剖宫产术中产后出血患者采用子宫横向环形压迫缝合术,显效56例,子宫切口出血立即减少,有效3例,15 min内出血减少,总有效率100%,其中3例术中同时行B-lynch缝合术。(2)出血量:所有患者出血量都控制在≤1000 ml。(3)不良反应:59例患者中1例出现1种不良反应,发生率为1.69%,一般在72 h内可缓解。结论子宫横向环形压迫缝合术可有效扼制子痫前期、妊娠期高血压、妊娠期糖尿病、双胎妊娠、疤痕子宫等因素导致的子宫收缩乏力性剖宫产术中产后出血的治疗和预防,同时适用于前置胎盘、胎盘植入、胎盘粘连、胎盘低置等胎盘因素致产后出血以及血小板减少等凝血因素异常引起产后出血的治疗和预防。具有高效、安全、方便、迅速廉价等优点,可作为预防和治疗剖宫产术中术后出血治疗的首选术式,值得临床推广应用。Objective：To analyze the clinical effect of transverse compression suture in the treatment and prevention of Post Cesarean section bleeding. Methods： To investigate the clinical efficacy of uterine lateral compression ring suture bleeding in the treatment and prevention of cesarean section postpartum, we did a retrospective study and ehoosed 59 cases of postpartum hemorrhage patients as the research object during the period from May 2016 to September 2016 in Tailm Cen- tral Hospital. Results ： （~）We found the curative effect of postpartum hemorrhage during cesarean section in patients with u- terine annular transverse compression suture, 56 cases markedly reduced uterine incision bleeding immediately, 3 cases ef- fective reduced the hemorrhage in 15min, the total efficiency of 100%, of which 3 cases were performed at the same time B-lynch suture. @）The amount of bleeding： All patients bleeding is controlled in 1000ml. （~）Adverse reactions： 59 cases of patients with 1 cases of adverse reactions, the incidence rate of 1.69%, and this condition can be alleviated generally with- in 72h. Conclusion ： Uterine transverse compression suture can effectively inhibit the ring of preeclampsia, gestational hy- pertension, gestational diabetes mellitus, twin pregnancy, prevention and treatment of uterine atony bleeding of cesarean section after the middle lead to uterine scar and other factors, also effective for placenta previa, placental implantation, pla- cental adhesion, placenta lower and other placental factors caused by postpartum hemorrhage and thrombocytopenia caused by abnormal blood coagulation factors in the prevention and treatment of postpartum hemorrhage. Uterine transverse com- pression suture can inhibit postpartum hemorrhage with high efficiency, safety, convenient, rapid and inexpensive, and it can be used for prevention and treatment after cesarean section postpartum hemorrhage preferred surgical treatment, worthy of clinical application.
