摘  要:目的 1470 nm半导体激光对大体积前列腺患者疗效评价及相关术式效果比较。方法选取郑州市第一人民医院2015-01—2016-12间收治的40例大体积良性前列腺增生症患者40例,经直肠前列腺超声测得前列腺体积80~210 m L,平均体积(132.4±20.7)m L。分别应用经尿道电切(对照组,n=20)和1470 nm半导体激光(观察组,n=20)对40例良性前列腺增生患者行手术治疗。评价两组手术时间、术中出血量、留置尿管时间及术后住院时间,术前、术后3个月国际前列腺症状评分、生命质量评分、最大尿流率相关指标的差异。结果 40例手术均顺利完成,两组手术时间差异无统计学意义;观察组较对照组术中出血量明显减少,术后留置尿管时间、住院时间均较对照组短,差异有统计学意义(P<0.05)。术后3个月观察组国际前列腺症状评分、生命质量评分、最大尿流率均较术前改善,手术前后差异有统计学意义(P<0.05)。结论 1470 nm半导体激光汽化术治疗良性前列腺增生安全有效,尤其对于大体积前列腺手术出血量少,愈合快,预后效果显著。Objective To compare the clinical therapeutic effect of transurethral resection of prostate of 1470 nm laser resection of prostate and transurethral resection of prostate( TURP). Methods 40 BPH patients suffering from bladder outlet obstruction with large prostate( 80 ~ 210 m L) were randomly divided into two groups: observation group( 1470 nm semiconducting laser,20 cases)and control group( TURP,20 cases). The clinical data in operative period of two groups were especially compared and analyzed.Results All patients are operated successfully and followed up for 3 months. There is no significant difference in operation time of two groups. Through comparing with the TURP group,the laser group has less blood loss,and less time of indwelling catheter and hospital stays( P〈0. 05). The international prostate symptom score( IPSS),quality of life score( QOL) and maximum urinary flow rate( Qmax) of the laser group after 3 months improve( P〈0. 05). Conclusion 1470 nm semiconducting laser is a new method treating patients suffering from bladder outlet obstruction with large prostate. It is safe and effective. It is also more dominant with less blood loss,less time of indwelling catheter and hospitalization stays.
