摘　　要：目的分析艾滋病病毒(HIV)抗体不确定者的检测结果,探讨HIV抗体不确定结果出现的规律和HIV不确定人群蛋白印迹试验(WB)带型的变化特征。方法对驻马店市HIV确证实验室2013-2015年报告的55例HIV抗体不确定人群进行跟踪随访,回顾性研究其WB带型及HIV酶联免疫吸附试验(ELISA)的S/CO值特征,观察其最终临床转归过程中的发展和变化。结果 2013-2015年驻马店地区共检测HIV抗体不确定者55例。经随访检测,有30例(54.5%)确证为阳性,21例(38.2%)确证阴性,4例(7.3%)失访。WB条带的组合模式共有18种。单次带型分析显示,p24带型出现26次,占总条带数的23.0%;其次是gp160,占总带数的19.5%。HIV筛查试验ELISA不同S/CO值样本的阳转率差异有统计学意义(χ~2=16.259,P=0.006)。结论驻马店市近几年发现HIV不确定结果中,env带的出现及HIV筛查试验ELISA的S/CO>6.0预示HIV感染的可能性较大,对HIV感染的判断应结合流行病学史及实验室相关检测手段进行综合诊断。Objective To analyze the uncertain results of HIV antibody detection confirmed by western blot（WB）,and to know their features of WB band pattern.Methods 55 cases with uncertain results of HIV antibody detection by the confirmation laboratory of Zhumadian city from 2013 to 2015,were followed up and observed for their development and changes of the final clinical outcome.The characteristics of S/CO of enzyme-linked immunosorbent assay（ELISA）and WB band were studied retrospectively.Results Out of the 55 cases of HIV antibody uncertainty,30（54.5%） cases were confirmed as HIV positive,and 21（38.2%）cases as negative.4 cases were lost for follow up.There were 18 combinations of WB bands.Single band analysis showed that p24 appeared 26 times,accounting for 23.0% of the total bands,and gp160 accounting for 19.5% of the total.The S/CO6.0 groups of the HIV screening test were significantly higher than the S/CO6.0 groups（χ~2=17.860,P0.0001）.Conclusion The emergence of env together with ELISA S/CO6.0 indicates the greater probability of HIV infection for those people with HIV uncertain results by confirmation WB testing.Comprehensive diagnosis of HIV infection should be considered based on the history of epidemiology and laboratory-related detections.
