两种试剂在HIV确证试验中的比较

摘　　要：目的为减少艾滋病病毒(HIV)检测中不确定结果的产生,缩短窗口期,寻找一种更准确、灵敏度更高的试剂。方法对已检测的9份HIV早期感染者的血清,及其阳转后的血清,分别用两种HIV抗体确证试剂盒检测并进行比较。结果 MP-WB试剂对9份HIV早期感染标本的检测结果均为HIV-1抗体不确定,RIBA试剂的检测结果为7份HIV-1抗体阳性,2份HIV-1抗体不确定。两种确证试剂对早期感染标本检测时,条带的主要区别在于是否检测出gp41带。两种确证试剂对9份阳转后血清的检测结果均为HIV-1抗体阳性。对RIBA与MPWB试剂的一致率和对gp41带检测能力进行McMemanr检验,差异有统计学意义(P=0.016,P=0.008)。结论与目前常用的MP-WB相比,RIBA试剂可以缩短HIV抗体确证实验的窗口期,减少不确定结果的产生。Objective To reduce the uncertain results in HIV detection,and shorten the window period in diagnosis of HIV-1 infection by using accurate and sensitive reagent.Methods The sera of 9 patients with early HIV infection were tested with two kinds of HIV antibody confirmation reagent kits.Results The results with MP-WB detection of 9 samples of early HIV infection were HIV-1 antibody uncertain,while RIBA test results had 7 samples of HIV-1 antibody positive,and 2 samples uncertain.The two kinds of reagents were used for confirmatory tests after the nine cases got their seroconversion to HIV positive and the results were all confirmed as HIV positive.The difference between RIBA and MP-WB（P=0.016） was significant,esp.in detection of early infection specimens to see whether the gp41 bands were detected. Conclusion Compared with the commonly used MP-WB,RIBA can shorten the window period of HIV antibody confirmation,and reduce the uncertain results.
