个案管理模式在艾滋病抗病毒治疗中的应用及效果

摘　　要：目的分析个案管理模式在艾滋病抗病毒治疗(ART)中的作用和效果。方法对2013年1月至2016年12月,在北京佑安医院艾滋病确证实验室确证的HIV抗体阳性者,以及在该院性病艾滋病门诊接受ART的病人,比较施行个案管理模式前后,ART的治疗率、服药依从性、病毒抑制率及队列维持率。结果个案管理实施后,接受ART的病人治疗率由个案管理实施前的86.86%提高到93.60%;ART的完全依从性从96.33%提高到99.00%,部分依从性由2.12%下降到0.83%,不依从者由1.55%下降到0.17%;病毒抑制率由96.33%提高到99.00%,病毒学失败率由3.67%下降到1.00%。结论个案管理模式的应用使接受ART的病人数增加,失访人数、转出人数、死亡人数及停药人数下降,对提高ART后病毒抑制率及抗病毒的队列保持率有一定的促进作用。Objective To study the role of case management in antiretroviral therapy.Methods HIV-1 infected patients receiving antiretroviral therapy（ART）at Beijing Youan hospital during Jan 2013-Dec 2016 were included in this study.Rates of antiretroviral therapy,treatment adherence,viral suppression and cohort maintenance were compared between patients treated before and after the initiation of case management.Results After the initiation of case management,the rate of antiretroviral therapy increased from 88.35% to 95.97%;the rate of adherence increased from 96.35%to 99.0%;the rate of viral suppression increased from 96.35% to 99% and the rate of cohort maintenance increased significantly as well.Conclusion The initiation of case management is beneficial to increase the rates of antiretroviral therapy,treatment adherence,viral suppression and cohort maintenance significantly and plays an important role in expanding antiretroviral therapy.
