摘 要:目的:对桡神经损伤患者的神经肌电图进行分析,探讨其在诊断桡神经损伤中的作用。方法:选择兵团医院2013年9月~2016年9月期间门诊及住院的30例桡神经损伤患者,采用肌电诱发电位仪对患者进行正中神经、尺神经和桡神经的运动及感觉神经传导测定及拇短展肌、小指展肌和指总伸肌针刺肌电图测定,观察所测运动和感觉神经的传导速度及所测肌肉的插入电位、异常自发电位、运动单位电位的波幅、时限和大力收缩时的募集电位。结果:30例桡神经损伤患者的桡神经运动潜伏期延长、传导速度减慢及诱发波幅降低,与正常参考值比较差异有统计学意义;而桡神经感觉神经损害早期绝大多数表现为感觉诱发波幅降低。针刺肌电图检查发现30块指总伸肌中73%出现自发电位,57%出现运动单位时限增宽大于20%,波幅增高大于70%。结论:神经肌电图检测能够为桡神经损伤患者的临床早期诊断、治疗及预后提供可靠依据。Objective:The electromyogram (EMG) of 30 patients with radial nerve injury was ana- lyzed to evaluate the role of electromyography in the diagnosis of radial nerve injury. Methods: 30 patients with radial nerve injury were selected from September, 2013 to September, 2016. Electromyographic e- voked potentials were used to measure the motor and sensory nerve conduction velocity and electromyo- gram of the limbs. The measured muscle movement unit potential of the waveform, amplitude, time limit and strong contraction of the recruitment potential were observed. Results:30 cases of radial nerve injury in patients with radial nerve motor latency, conduction velocity slowed down and evoked amplitude de- creased, and these data were statistically significant, compared with the normal value, while the radial nerve sensation was only manifested as decrease in evoked amplitude. According to EMG 73.3% sponta- neous potential wsa found in 30 cases of extensor digitorum communis, 56.7 % of the movement unit time limit was more than 20 %, the amplitude increased by more than 70 %. Conclusion.. EMG examination may provide reliable evidence for clinical diagnosis on the early stage, treatment and prognosis.
