基于文献分析的浆细胞性乳腺炎各期辨证要点

摘　　要：目的:探讨浆细胞性乳腺炎各期辨证要点,以求指导临床治疗。方法:通过文献分析,分析浆细胞性乳腺炎各期用药特点,运用"以方测证"的方法来探求其各期证型。结果:1996—2015年间,按照纳入标准和排除标准共纳入文献23篇。肿块期共有22张处方,其中使用频率最高的中药是柴胡,按功效分类,使用频率最高的是清热解毒药,其次是活血止痛药,寒凉药占60.20%;脓肿期共有12张处方,其中使用频率最高的中药是金银花,按功效分类,使用频率最高的是清热解毒药,其次是补气药,寒凉药占70.33%;瘘管期共有16张处方,其中使用频率最高的中药是当归,按功效分类使用频率最高的是清热解毒药,其次是补血药,寒凉药占44.64%;围手术期共有5张处方,其中使用频率最高的中药当归,按功效分类,使用频率最高的是补血药,其次是清热解毒药,寒凉药占62.06%。结论:肿块期的主要辨证为肝气郁滞,热毒炽盛;脓肿期多为热毒壅盛,气血两虚;瘘管期主要辨证为热毒未清之证;围手术期多为气血两虚之证。Objective：To explore syndrome differentiation key points of plasma cell mastitis so as to guide the clinical therapy.Methods：Through literature analysis,analyze plasma cell mastitis medical characteristics in each stage by using the method of＂speculating the symptoms by means of prescriptions＂to explore its syndromes of stages.Results：A total of23 papers were included from 1996 to 2015,according to the inclusion criteria and exclusion criteria.The most frequent herb applied was Chaihu（Radix Bupleuri）.The most frequent herbs applied are herbs of clearing heat and removing toxicity.More frequent herbs are herbs of activating blood and removing pathogenic heat from blood according to effect.The herbs of cold and cool accounted for 60.20% in the 22 prescriptions used for the lump stage of plasma cell mastitis.The most frequent herb applied are Jinyinhua（Lonicera japonica）.The most frequent herbs applied are herbs of clearing heat and removing toxicity.More frequent herbs are herbs of invigorating qi according to effect and the herbs of cold and cool accounted for 70.33% in the 12 prescriptions used for the abscess stage of plasma cell mastitis.The most frequent herb applied is Danggui（Angelica sinensis）.The most frequent herbs applied were herbs of clearing heat and removing toxicity.More frequent herbs were herbs of invigorating blood according to effect and the herbs of cold and cool accounted for 44.64% in the 16 prescriptions used for the fistulat stage of plasma cell mastitis.The most frequent herb applied was Danggui（Angelica sinensis）.The most frequent herbs applied were herbs of activating blood and more frequent herbs were herbs of clearing heat and removing toxicity according to effect.The herbs of cold and cool accounted for 62.06% in the 5 prescriptions used for perioperative period.Conclusion：Liver Qi stasis and toxic heat flourishing are the syndrome type of lump stage and the exuberance of heat-toxin and qi-blood deficiency are the syndrome type of abscess stage.Leaving toxi
