易化牵伸术结合软组织疗法治疗创伤性肩周炎患者的疗效观察

摘　　要：目的：观察易化牵伸术结合软组织疗法治疗创伤性肩周炎的临床疗效.方法：将64例创伤性肩周炎患者采用随机数字表法分为对照组和观察组,每组32例.对照组患者接受软组织疗法和常规康复治疗;观察组接受易化牵伸术结合软组织疗法和常规康复治疗.每组患者每天接受2次治疗,持续4周.分别于治疗前、治疗4周后,采用欧洲肩关节协会Constant-Murley肩关节评定方法（CMS）[1]进行疗效评估.结果：经过4周的治疗,对照组、观察组患者的肩关节主观评价分数（包括疼痛和功能活动,满分为35分）由治疗前的10.81±4.97分和10.75±3.73分,分别提升至15.88±5.78分和21.88±3.83分,治疗前后差异均有统计学意义（P〈0.05）,且观察组患者的改善程度显著优于对照组（P〈0.05）.2组患者治疗4周后的肩关节客观评价分数（包括肩关节活动度和肌力,满分65分）由治疗前的29.19±12.23分和26.31±13.02分,分别提升至42.06±12.57分和46.81±10.23分,治疗前后差异均有统计学意义（P〈0.05）,且观察组患者的肩关节活动度改善程度显著优于对照组（P〈0.05）.结论：易化牵伸术结合软组织疗法能更好地改善创伤性肩周炎患者的疼痛症状,并能促进患者肩关节活动度的增加.Objective： To observe the facilitated stretching combined with soft tissue therapy in the treatment of traumatic clinical cura- tive effect of traumatic periarthritis of shoulder. Methods： 64 patients with traumatic periarthritis of shoulder using the random number ta- ble method was divided into matched group and stretching group with 32 cases in each group. The matched group patients were treated with soft tissue therapy and routine rehabilitation therapy; patients in the stretching group were treated with facilitated stretching combined with soft tissue therapy and routine rehabilitation therapy. Patients of each group were treated twice per day, for 4 weeks. The European associ- ation of shoulder joint Constant - Murley shoulder joint（CMS） was adopted to evaluate clinical effects before and at the end of treatment. Results： After 4 weeks of treatment, all patients＇CMS scores improved significantly （P 〈0.05）. Shoulder score of subjective evaluation and objective evaluation achieved a significantly greater improvement in the stretching group than in the matched group. Conclusion： Fa- cilitated Stretching combined with soft tissue therapy could effectively relieve pain and increase the shoulder range of motion （ROM） for traumatic periarthritis of shoulder.
