摘 要:目的探讨士兵在训练准备期与训练后免疫功能的变化情况,指导军训计划的制订。方法采用完全随机分组的方式将某部队刚入伍士兵分为高强度组和低强度组(各80名),低强度组进行常规军事训练,高强度组则给予高强度军事训练内容,分别在训练准备期及训练第7、14、28天测定2组的血常规、免疫球蛋白(immunoglobulin,Ig)G、IgA、IgM以及T细胞亚群等免疫指标,并观察2组上呼吸道感染的发生情况。结果训练前2组各项指标比较差异无统计学意义(P>0.05);2组在训练后CD4+T细胞、CD8+T细胞、CD4+/CD8+及IgM水平未见显著变化;训练后2组白细胞计数、中性粒细胞、IgG、IgA呈下降趋势,而且高强度组下降趋势更加明显;训练第14、28天,2组白细胞计数差异均有统计学意义(P<0.05);训练第7、14、28天,2组的中性粒细胞和IgG指标差异均有统计学意义(P<0.05);训练前2组上呼吸道感染发生率差异无统计学意义(P>0.05),训练后高强度组上呼吸道感染的发生率显著高于低强度组(P<0.05)。结论对于士兵,合理的军事训练使机体的免疫功能得到显著提高,抗病能力也有所增强;但过高强度的军事训练会在某种程度上抑制免疫系统功能,造成免疫功能的降低。Objective To investigate the changes of immune function in soldiers during preparation and after training,guide the development of the implementation plan of military training.Methods An army of the newly enlisted soldiers were completely randomized into high-intensity group,and the low-intensity group,and each group contained 80 soldiers.Low-intensity group soldiers went through routine military training,while high-intensity group soldiers were treated with high intensity military training content.Blood test,immunoglobulin(Ig) G,IgA,IgM and T cell subsets and other imm-une parameters of the two groups were observed in military training in preparation,7 d,14 d,28 d of the training respectively,and the incidence of upper respiratory tract infections were also compared.Results There was no significant difference between the two groups before the training in all indicators(P〈0.05).There was no significant difference in the levels of each T cell subsets,CD4+/CD8+and IgM between the two groups including early post-training and late post-training.The white blood cell count,neutrophil percentage,IgG,IgA of the two groups decreased after training,while white blood cell count(day 14,day 28),neutrophil percentage(day7,day 14,day 28),IgG levels(day 7,day 14,day 28) of the high-intensity group were significantly lower than those of the low-intensity group(P〉0.05).There was no significant difference before training and after training in the incidence of respiratory infections(P〉0.05),and the incidence of respiratory infections of the high-intensity group was significantly higher than that of the low intensity group soldiers(P〈0.05).Conclusion For the soldiers,legitimate military training can significantly improve the body's immune function,and enhance disease resistance,but the high intensity of military training will suppress the immune system in some way,resulting in reduced immune function.
