清热凉血解毒方药浴联合窄谱中波紫外线照射治疗白疕(血热证)70例

摘　　要：目的观察口服清热凉血解毒中药及药浴联合窄谱中波紫外线(NB-UVB)照射治疗寻常型银屑病(白疕,血热证)的临床疗效。方法选择70例白疕(血热证)患者,随机分为2组。对照组35例口服银屑灵(膏剂)和NB-UVB照射治疗,治疗组35例在此基础上加用清热凉血解毒中药药浴治疗。观察皮疹症状(红斑、鳞屑、浸润程度)和皮疹面积的变化,以银屑病皮损程度评分(PASI评分法)进行评估,比较疗效。结果治疗第6,10周后,PASI评分组内比较,均有显著降低,2组间比较,差异也均有统计学意义(P<0.05)。同时,第6周治疗组与对照组有效率分别为82.85%和60.00%,第10周分别为91.43%和71.42%,组间比较差异均有统计学意义(P<0.05)。结论口服清热凉血解毒中药及药浴联合NB-UVB照射治疗白疕(血热证)疗效可靠。Objective To observe the clinical effect of oral traditional Chinese medicine which has the function of clearing heat-toxin and cooling blood, as well as medicated bath combined narrow spectrum UVB rays （NB-UVB） irradiation therapy in the treatment of blood heat syndrome of psoriasis vulgaris. Methods 70 cases patients with psoriasis vulgaris （blood heat syndrome） were randomly divided into treatment group （n = 35） and control group（ n = 35）. The control group was given tigason （paste） and NB-UVB irradiation therapy, while the treatment group was given clearing heat-toxin and cooling blood herbs bath. The skin rash symptoms, including erythema, squama and infiltrated degree, as well as the rash area and Psoriasis Area and Severity Index were evaluated to compare the effect. Both groups were taken decocted extract, the treatment group given medicated bath combined narrow spectrum UVB rays （NB-UVB） light therapy, the control group given narrow spectrum UVB rays （NB-UVB） irradiation therapy. Results On sixth and tenth weeks after treatment, PASI score decreased significantly than be- fore treatment, and significant difference showed between two groups （P 〈 O. 05）. Meanwhile, on sixth week of treatment, the effective rate of treatment group and control group were 82.85% and 60.00% , separately, and on tenth week, they were 91.43% and 71.42% , with statistical difference between groups （P 〈 O. 05 ）. Conclusion Oral traditional Chinese medicine which has the function of clearing heat- toxin and cooling blood, as well as medicated bath combined NB -UVB irradiation therapy has good effect in the treatment of blood heat syndrome of psoriasis vulgaris.
