6-Sigma管理模提高胃癌胃大部分切除患者的护理满意度

摘　　要：目的探讨6-Sigma管理模式在胃癌胃大部分切除术后患者护理中的应用效果,分析该方法对患者生理指标、护理质量及成本管理的影响。方法选择在我院接受胃癌胃大部分切除术后的200例患者,其中100例作为观察组适用6-Sigma管理模式,另100例作为对照组采用常规管理方式;比较两组患者不同时间段的疼痛、焦虑、生理指标、护理质量、满意度、住院时间、住院费用。结果观察组患者在术后第5天及出院前1 d的疼痛及焦虑评分均低于对照组(P<0.05)。观察组与对照组患者满意度(93%vs 81%),护士满意度(95%vs 76%),护理质量评分(92.32±5.29分vs 80.41±6.34分)有统计学差异。观察组住院时间少于对照组(13.5±3.1 d vs 17.2±4.6 d,P<0.05)。观察组住院总费用24588.3±4282.2元,单日费用1823.2±323.6元,费用均明显低于对照组。结论 6-Sigma管理有助于缓解胃癌大部分切除术后患者的焦虑情绪,减轻疼痛,有利于改善护理质量,减少住院时间与住院费用,增强患者与护士满意度。Objective To explore the effect of 6 sigma management model on nursing in patients with gastric cancer after gastratrophia with physiological index, nursing quality and cost management. Methods A total of 200 patients who underwent subtotal resection of gastric cancer in our hospital were selected.In which 100 patients were used as control group with routine management method and the other were treated as observation group with 6-sigma management model. Pain, anxiety and physiological indexes of the patients in different time periods were compared. Nursing quality, satisfaction, hospitalization time and hospitalization costs were observed. Results After performing 6-sigma, pain and anxiety scores of the observation group were lower than that of control group at the fifth day after operation and 1 d before discharge. In observation group, patients＇ satisfaction （93%）, nurses＇ satisfaction （95%） was significantly higher than that in control group.Hospital stays （13.5±3.1） in observation group was shorter than that in control group. Total hospital expenses （24588.3±4282.2）, daily hospital expenses （1832.2±323.6） were significantly lower in observation group than control group. Conclusion Six-sigma management is beneficial for relieving anxiety of patients with gastric cancer resection. It can reduce pain, improve nursing quality, cut down hospital stay and hospitalization expenses and enhance patients and nurses＇ satisfaction.
