前颞叶离断术治疗颞叶癫痫的疗效分析

摘　　要：目的探讨前颞叶离断术(anterior temporal lobotomy,ATL)治疗药物难治性颞叶癫痫的可行性和疗效。方法自2015年10月至2016年10月,采用前颞叶离断术治疗药物难治性颞叶癫痫22例,患者均经过详细的术前评估检查,手术指征和标准前颞叶切除术相同,所有患者经过多科会诊讨论确定。结果 22例患者均行前颞叶离断术,手术过程顺利,围手术期平稳度过。术后对侧肢体运动和言语功能等和术前无明显变化。术后随访6~18个月,17例患者疗效为EngelⅠ级,5例为EngelⅡ级。结论前颞叶离断术是颞叶癫痫手术方式的重要改良,疗效确切,术后不遗留残腔,术后恢复过程顺利。Objective To investigate the practicability and outcome of anterior temporal lobotomy（ ATL） for drug resistant temporal lobe epilepsy. Methods From Oct. 2015 to Oct,2016,there are 22 drug ressistant epilepsy patients underwent ATL in Dept. of Neurosurgery of Peking Union Medical College Hospital. All patients were carefully evaluated and diagnosed as temporal lobe epilepsy before surgery. Patient selection and evaluation was the same to that for other patients undergoing anterior temporal lobectomy. Results All patients tolerate the procedure well and the post operation period was smooth. No significant neurological deficits were found. In the follow-up duration of 6 - 18 months,17 patients were seizure free,5 had rare seizures（ Engel II）. Conclusion Anterior temporal lobotomy may produce results equivalent to anterior temporal lobectomy in the control of drug resistant temporal lobe epilepsy. ATL is an acceptable alternative surgical technique for patients with intractable temporal lobe epilepsy.
