血清DHT和bcl-2水平与前列腺增生疗效的关系

摘　　要：目的探讨血清双氢睾酮(DHT)和B淋巴细胞瘤-2(bcl-2)水平与前列腺增生(BPH)经尿道前列腺电切术(TURP)近远期疗效的关系。方法以拟行TUPKP治疗的BPH患者76例为研究组,检测比较患者术前、术后1个月和6个月的血清DHT和bcl-2水平及同期国际前列腺症状评分(IPSS)、生活质量评分(QOL)和最大尿流率(Qmax)。并以同期基线资料匹配的健康查体志愿者30例为对照组。比较两组血清DHT和bcl-2水平、IPSS得分、QOL得分和Qmax。采用Pearson线性相关分析法分析研究组术前血清DHT和bcl-2水平与其围术期IPSS得分、QOL得分和Qmax的关系。结果与对照组比较,研究组术前血清DHT和bcl-2水平、IPSS得分和QOL得分均升高而同期Qmax降低,差异均有统计学意义(P<0.05)。与术前比较,研究组术后1个月和6个月的血清DHT和bcl-2水平、IPSS得分和QOL得分均降低而同期Qmax则均升高;与术后1个月比较,研究组术后6个月的血清DHT和bcl-2水平、IPSS得分和QOL得分均降低而同期Qmax则均升高,差异均有统计学意义(P<0.05)。Pearson线性相关分析结果显示,研究组术前血清DHT和bcl-2水平与其术前IPSS得分、QOL得分和Qmax均密切相关(DHT:r=0.862、0.895、-0.879;bcl-2:r=0.822、0.873、-0.855,P<0.05),与其术后1个月IPSS得分、QOL得分和Qmax均密切相关(DHT:r=0.825、0.873、-0.818;bcl-2:r=0.815、0.823、-0.868,P<0.05),与其术后6个月IPSS得分、QOL得分和Qmax亦均密切相关(DHT:r=0.805、0.828、-0.835,bcl-2:r=0.836、0.851、-0.879,P<0.05)。结论 BPH TURP术前血清DHT和bcl-2水平与病情及近远期疗效均密切相关,可能作为其病情和TURP近远期疗效评估的参考指标。Objective To investigate the relationship between serum dihydrotestosterone （DHT） and B cell lymphoma -2 （bcl-2）levels and the short and long-term effect of prostatic transurethral resection（TURP） treatment for patients with benign prostate hyperplasia（BPH）. Methods 76 cases of patients with BPH and had TURP treatment were selected as the study group,preoperative and postoperative 1 month and 6 months serum DHT and bcl-2 levels and the same International Prostate Symptom Score（IPSS）,quality of life score（QOL）and maximum urinary flow rate（Qmax）of the patients were detected and compared. And 30 cases of healthy volunteers with matched baseline data and had physical exam in the same period were selected as the control group. The serum DHT and bcl- 2 levels,IPSS score,QOL score and Qmax of the two groups were compared. The relationship between preoperative serum DHT and bcl-2 levels with perioperative IPSS score , QOL score and Qmax of the patients in the study group were analyzed by Pearson linear correlation analysis. Results Compared with the control group,serum DHT and bcl-2 levels,IPSS score and QOL score of the study group were higher while in the same period Qmax of the study group was lower （P〈0.05）. Compared with the preoperative ,postoperative 1 month and 6 months serum DHT and bcl-2 levels,IPSS score and QOL score of the study group were decreased while Qmax of the study group in the same time was increased. Compared with postoperative 1 month,postoperative 6 months serum DHT and bcl-2 levels,IPSS score and QOL score of the study group were decreased while Qmax of the study group in the same time was increased （P〈0.05）. Pearson linear correlation analysis results showed that ,preoperative serum DHT and bcl-2 levels of the study group with preoperative IPSS score,QOL score and Qmax were closely related（DHT：r=0.862, 0.895,-0.879;bcl-2：r=0.822,0.873,-0.855,P〈0.05）,postoperative 1 month IPSS score,QOL score and Qmax were closely related（DHT：r=0.825,0.873,-0
