G-CSF动员内皮祖细胞对心肌梗死患者心功能的影响

摘　　要：目的 探讨粒细胞集落刺激因子动员内皮祖细胞对心肌梗死患者心功能的影响.方法 将90例心肌梗死患者按随机数字表法分为A组和B组,每组45例,均予以常规药物对症治疗,B组联合粒细胞集落刺激因子动员治疗,观察4个月.于治疗前及治疗后第7 d、4个月采用荧光双染色法对外周血进行内皮祖细胞检测,于治疗前后检测两组左心室射血分数和6 min步行实验,随时记录治疗过程中出现的不良反应.结果 B组患者应用粒细胞集落刺激因子动员治疗后第7 d,患者外周血内皮祖细胞较治疗前显著升高（P〈0.01）,到治疗4个月时内皮祖细胞又恢复到治疗前水平.治疗后两组左心室射血分数、6 min步行实验指标均较治疗前显著升高（P〈0.05或0.01）,但B组的升高幅度明显大于A组（P〈0.05）.A组总有效率为71.1%,B组为88.9%,B组总有效率显著高于A组（P〈0.05）.治疗过程中两组均未观察到异常不良反应.结论 粒细胞集落刺激因子高效动员内皮祖细胞后可以显著改善心肌梗死患者心功能,临床疗效显著,安全性高,值得临床推广应用.Objective To investigate the effects of G-CSF on cardiac function of myocardial infarction patients by mobilizating endothelial progenitor cells. Methods A total of 90 MI patients were selected and assigned to group A, B according to random number table of 45 ones each. They were all given conven- tional medical treatment, group B plus G-CSF mobilization, and were followed 4 months. EPCs was detected by fluorescence analysis before and 7d, 4 months after treatment. The LVEF and 6-min walk test were detected before and after treatment. And the adverse reactions during therapeutic process were also recor- ded. Results 7d after treatment, EPCs of group B was significantly increased after G-CSF treatment （P〈0.01）. After 4 month, EPCs was decreased to base line （P〈0.01）. LVEF and 6MWT of the two groups after treatment were significantly increased （P〈0.05 or 0.01）, but ascending range of group B was obviously larger than group A （P〈0. 05）. Total effective rate of group B （88.9%） was obviously higher than group A （71.1%） （P〈0.05）. Abnormal adverse reactions of the two groups during treatment were not observed. Conclusion G-CSF can significantly improve heart function in MI patients by mobilizating EPCs, it has significant clinic effect, higher safety and is worthy of clinic promotion.
