不同方案治疗宫颈高危型HPV感染合并慢性宫颈炎临床研究

摘　　要：目的探讨氯喹那多普罗雌烯阴道片与奥平栓两种方案治疗宫颈高危型人乳头瘤病毒感染合并慢性宫颈炎的临床疗效。方法将90例高危型人乳头瘤病毒感染合并慢性宫颈炎患者按随机数字表法分为观察组与对照组，每组45例。观察组给予氯喹那多-普罗雌烯阴道片治疗，对照组给予奥平栓治疗。比较两组患者临床疗效、阴道排液量、脱痂止血时间及创面愈合时间，于治疗后3个月、6个月及12个月末比较两组人乳头瘤病毒-DNA转阴率。结果观察组总有效率为95．6％，对照组为82．2％，观察组显著高于对照组（P〈0．05）；观察组阴道排液量显著少于对照组（P〈0．05），脱痂时间及创面愈合时间均显著短于对照组（P〈0．05或0．01）；治疗6个月末起观察组人乳头瘤病毒-DNA转阴率显著高于对照组（P〈0．01）。结论氯喹那多普罗雌烯阴道片治疗高危型人乳头瘤病毒感染合并慢性宫颈炎疗效优于奥平栓，可以显著提高转刚率，增强机体免疫功能，值得临床推广应用。Objective To investigate the clinic effect of Chlorquinaldol/Promestriene vaginal tablets and opin suppository in treating cervical high-risk HPV infection combined with chronic cervicitis. Methods 90 cases with cervical high-risk HPV infection combined with chronic cervicitis were selected and divided into observation group and control group, 45 cases in each group. Observation group was given Chlorquinaldol/Promestriene vaginal tablets, control group was given opin suppository. Clinic effect, va ginal discharge, decrustation and hemostatic time, wound healing time of the two groups were compared, after treatment of 3, 6, 12 months, negative conversion ratio of HPV-DNA of the two groups were corn pared. Results Total efficiency of observation group （95.6%） was significantly higher than control group （82.2%） （P〈0.05）; vaginal discharge of observation group was significantly lower than control group （P〈0.05）, decrustation time and wound healing time were all significantly shorter than control group （P〈0.05 or 0.01）; after treatment of 6 months, negative conversion ratio of HPV-DNA of observation group was significantly higher than control group （P〈0.01）. Conclusion Compared with opin suppository, chlorquinaldol promestriene vaginal tablets is proved to be more effective in treating cervical high risk HPV infection combined with chronic cervicitis, which significantly improve the total curative effects and negative conversion rate and enhance the body immunity. Thus, it deserves to be widely promoted in clini cal field.
