Ⅲ度烧伤创面早期削痂后保留健康脂肪组织大张皮移植临床研究

摘　　要：目的探讨Ⅲ度烧伤创面早期削痂后保留健康脂肪组织大张皮移植的临床效果。方法对48例Ⅲ度烧伤创面早期削痂患者治疗过程评估并保留健康脂肪组织，对所有患者分别行小皮片和大张皮移植，比较不同皮片移植后临床疗效。结果大张皮移植创面愈合时间、愈合优良率、术后第7d移植存活率均显著高于小皮片移植（P〈0．01）；远期疗效评估中大张皮移植优良率93．8％，小皮片移植优良率70．8％，大张皮移植优良率显著高于小皮片移植优良率（P〈0．01）。结论Ⅲ度烧伤创面早期削痂后保留健康脂肪组织行大张皮移植较小皮片移植效果好，患者术后创面愈合时间短，优良率、存活率高，远期疗效显著。Objective To study the clinical effects of preserving healthy adipose tissue after early excision of Ⅲ degree burn wounds by adopting large sheet skin grafting. Methods 48 cases of patients with early excision of Ⅲ degree burn wounds were selected as the study objectives. Therapeutic process was evaluated and healthy adipose tissues were preserved. All the patients were given small sheet skin grafting and large sheet skin grafting. Compare the clinical effects of both skin grafting. Results The wound heal- ing time, the excellent rate of wound healing, survival rate of 7d after surgery for the patients who were given large sheet skin grafting were significantly higher than those who were given the small sheet skin grafting （P〈0.01）; the excellent rate of long-term curative effects for the patients who were given large sheet skin grafting （93.8%） was significantly higher than those given small sheet skin grafting （70.8%） （P〈0.01）. Conclusion Preserving healthy adipose tissue after early excision of Ⅲ degree burn wounds by adopting large sheet skin grafting is proved to be more effective than adopting small sheet skin grafting. With short wound healing time, high excellent rate and survival rate and significant short-term curative effects, large sheet skin grafting is of great valuable in clinical field.
