无缝合集束化干预降低导管相关性血流感染的研究

摘　　要：目的探讨无缝合固定方法对中心静脉导管相关性血流感染(CRBSI)的影响,为临床治疗提供参考依据。方法选取2014年-2016年外科ICU、内科ICU、内科CCU入院后留置中心静脉导管≥2d的患者3 680例,2014年1月-2015年6月采用传统缝合固定导管集束化作为对照组1 730例,2015年7月-2016年12月采用无缝合固定导管集束化作为干预组1 950例,比较两组患者CRBSI的发生率。结果对照组患者千日导管感染率为4.08‰、高于对照组的1.54‰;对照组针刺伤发生15例,发生率为0.87%,干预组未发生针刺伤;对照组导管维护时间为(17.26±2.56)min,干预组导管维护时间为(12.00±1.65)min,经统计分析,两组患者间差异均有统计学意义(P<0.05)。结论无缝合集束化干预能有效降低针刺伤、缩短导管维护时间。OBJECTIVE To investigate the effect of no-suture fixation on central venous catheter-related blood stream infections（CRBSI）,so as to provide references for clinical treatment.METHODS A total of 3680 patients with central venous catheters（CVC）≥2dfrom surgical ICU,internal medicine ICU,and internal medicine CCU from 2014 to 2016were selected and divided into control group（from Jan.2014 to Jun.2015,n=1730）and intervention group（from Jul.2015 to Dec 2016,n=1950）.Patients of control group fixed their central venous catheters with suture,and patients of intervention group fixed their central venous catheters with adhesive plaster.The incidence of CRBSI was compared between the two groups of patients.RESULTS The incidence of CRBSI of control group was 4.08‰,which was higher than 1.54‰ of intervention group.There were 15 cases occurred needle injure in control group,with the incidence of 0.87%,and there was no needle injure in intervention group.Furthermore,time for cleaning catheter were（17.26±2.56）min in control group,and（12.00±1.65）min in intervention group,and the difference was significant（P〈0.05）.CONCLUSIONThe method of central line bundle for no-suture fixation can decrease needle injury,and shorten catheter cleaning time.
