摘 要:目的探讨脑卒中高危人群幽门螺杆菌(Hp)感染与血脂水平及促动脉硬化指数(AIP)的关系,为其血脂紊乱及动脉粥样硬化的防治提供新思路。方法连续性选择2014年6月至2015年1月皖北煤电集团总医院体检中心体检者2872例,收集其临床资料,筛选出脑卒中高危人群371例,采用14C尿素呼气试验检测Hp感染情况,同时采用酶联免疫吸附法测定血清氧化型低密度脂蛋白(ox-LDL)水平,分析Hp感染与总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDLC)、高密度脂蛋白胆固醇(HDLC)、ox-LDL及AIP之间的关系。结果 Hp感染者155例(41.8%,155/371),Hp感染组血清TC、TG、LDLC、ox-LDL、AIP水平明显高于非HP感染组(P<0.05)。多元线性回归分析显示,Hp感染与AIP水平呈正相关。结论 Hp感染可能在一定程度上引起脑卒中高危人群脂质代谢紊乱,导致AIP水平增高,促进动脉粥样硬化的进展。Aim To investigate the relationship among helicobacter pylori( Hp) infection with blood lipid level and AIP in patients with high risk of stroke,and to provide new ideas for the treatment of dyslipidemia and atherosclerosis.Methods Three hundred and sevety-one patients with high risk of stroke were screened from 2872 people who were consecutively selected and had health examination from June 2014 to January 2015 in the Department of Medical Center,General Hospital of Wanbei Coal and Electricity Group. Their clinical information was collected. The Hp infection status was detected by the 14 C urea breath test and the serum oxidized low density lipoprotein( ox-LDL) levels was determined by enzyme linked immunesorb-ent assay( ELISA). The relationship between Hp infection and total cholesterol( TG),triglyceride( TG),low density lipoprotein( LDLC),high density lipoprotein( HDLC),ox-LDL,AIP were analyzed.Results 155 patients( 41.8%,155/371) were found infected by Hp; the levels of TC,TG,LDLC,ox-LDL and AIP in Hp infection group were significantly higher than those in non HP infection group,the difference was statistically significant( P〈0.05). Multiple linear regression analysis showed that Hp infection was positively correlated with the level of AIP.Conclusion Hp infection may induce the disorder of lipid metabolism in high risk population of cerebral apoplexy,which leads to the increase of AIP level,and promotes the development of atherosclerosis.
