泪槽韧带释放联合眶脂重置矫治中老年性泪槽和睑颊沟凹陷

摘　　要：目的探讨矫治中老年性泪槽和睑颊沟凹陷的手术方法及疗效方法选择2010年7月至2015年11月中老年性眼袋伴有明显泪槽和睑颊沟凹陷的患者87例，采用肌皮瓣法下睑成形术，术中于眶下缘骨膜表面松解泪槽韧带和眼轮匝肌支持韧带复合体。并将眶隔脂肪释放平铺重置于此处骨膜上，根据松弛程度最大限度剪除部分眼轮匝肌及松弛皮肤结果所有受术音的泪槽和睑颊沟得到明显矫正，无下睑凹陷、睑外翻等并发症结论采用松解泪槽韧带和眼轮匝肌支持韧带复合体，释放并重置眶隔脂肪，对矫正老年性泪槽和睑颊沟凹陷取得满意效果，且简便安全。Objective To explore the surgical methods and efficacy of treatment in the tear trough deformity and palabromalar groove in elderly people. Methods Between July 2010 and November 2015, 87 cases who had lower eye baggy deformity with tear trough and palabromalar groove were treated by a transcutaneous lower blepharoplasty of skin-muscle flap procedure. The tear trough ligament and orbicularis muscle support ligament were stripped. The orbital fat was released and then reset over the orbital rim. The loose skin and orbicularis oculi muscle was excised and tightened. Results All patients with tear trough and palabromalar groove were collected. No lower eyelid depression, ectropion and other complications. Conclusion The technique of releasing the tear trough ligament and orbital fat reset can achieve satisfactory result in the correction of tear trough deformity and palabromalar groove in elderly people.
