META髓内钉与微创经皮钢板内固定技术治疗胫骨中上段骨折早期疗效分析

摘　　要：目的比较META髓内钉与微创经皮钢板内固定(MIPPO)技术治疗胫骨中上段骨折早期疗效差异。方法随机选取2011年1月至2015年3月分别采用META髓内钉(A组)与MIPPO技术(B组)治疗胫骨中上段骨折患者各20例,综合评估患者术中出血量、手术时间、术后骨折愈合时间,并通过评估骨折术后成角角度和对术后膝关节进行美国膝关节协会系统功能(AKSS)评分评价胫骨功能情况。结果 A组术中出血量少于B组,且手术时间短于B组,差异有统计学意义(P<0.05);两组患者其他指标比较,差异无统计学意义(P>0.05)。结论META髓内钉与MIPPO技术治疗胫骨中上段骨折均获得良好复位质量及下肢功能恢复,但在术中出血量及手术时间上,META髓内钉治疗优势明显,值得临床推广使用。Objective To comparatively analyze the early curative effect of the Meta intramedullary nailing and minimally invasive pereutaneous plate osteosynthesis （MIPP0） technique for treating the fracture of tibial upper middle section. Meth- ods A total of 40 patients with fractures of tibial upper middle section were received treatemnt in our hospital from January 2011 to March 2015 were selected. There were 20 cases which were treated by META intramedullary nailing in META group （Group A）and 20 cases which were treated by MIPPO technique in MIPPO group （Group B）. Postoperative healing time, intraoperative blood loss, the operation time and the limb function of these patients were assessed by evaluation of fracture angle, the degree of fracture healing and postoperative knee function （AKSS）. Results Totally 40 cases were followed up with a average time of（ 12. 8 ± 1.6） months ,and the operating time of META group was significantly less than MIPPO group （ P 〈 O. 05 ）, intraoperative blood loss of META group was also significantly less than MIPPO group（ P 〈 0.05 ）. There were no significant differences in postoperative heal time and the operation time （P 〉 0. 05 ）. Conclusion The way of the Meta intramedullary nailing for treatment of the fracture of tibial upper middle section can acquire better restoration quality and lower limb function recovery. This method is worthy of clinical promotion.
