个性化术前访视在达芬奇机器人胃癌患者围术期心理的影响

摘　　要：目的:探讨个性化术前访视对达芬奇机器人胃癌患者围术期心理的影响。方法:选择2016年11月-2017年4月本科住院手术的50例胃癌患者,按照随机数字表法分为观察组和对照组,各25例。对照组采用常规术前访视,观察组采用个性化术前访视。对比分析两组术前1 d、术前即刻焦虑及抑郁程度,术前1 d睡眠质量影响及术后3 d护理满意率情况。结果:两组访视前焦虑及抑郁评分比较,差异均无统计学意义(P>0.05);观察组访视后焦虑及抑郁评分均明显低于对照组(P<0.05)。观察组术前1 d睡眠质量、睡眠时间、睡眠效率、睡眠障碍、安眠药物、日间功能障碍及PSQI总分均明显好于对照组,差异均有统计学意义(P<0.05)。观察组访视后满意度明显高于对照组(P<0.05)。结论:个性化术前访视解除了患者对达芬奇机器人手术效果的顾虑,以积极乐观的态度面对疾病,缓解了患者心理压力,增强了患者战胜疾病的信心,使患者积极主动配合护理治疗并顺利完成手术。Objective： To investgate the effect of the individualized preoperative interview on the perioperative psychological of gastric cancer with Da Vinci robot surgery.Method： The data of 50 gastric cancer patients treated with Da Vinci robots surgery in our department from November 2016 to April 2017 were retrospectively analyzed, and they were randomly divided into the observation group and the control group, 25 cases in each group.The control group was treated with conventional preoperative visting and nursing, while the observation group was treated with individualized preoperative visiting and nursing.The anxiety and depression at 1 day pre-operation and immediately before operation, the quality of sleep at 1 day pre-operation and the satisfaction of the nursing at 3 days after operation of two groups were observed.Result： There were no statistical significances in the anxiety and depression score of two groups before visiting （ P〉0.05 ） .After visiting, the anxiety and depression score of the observation group were significantly lower than those of the control group （P〈O.05） .The preoperative 1 day sleep quality, sleep time, sleep efficiency, sleep disorder, daytime dysfunction and PSQI score of the observation group were significantly better than those of the control group （ P〈0.05 ） .Satisfaction of the nursing of the observation group after visiting was significantly higher than that of the control group （ P〈O.05 ） .Conclusion： Individualized preoperative visiting for gastric cancer patients with Da Vinci robot operation can effectively relieve the patients psychological pressure, enhance the patients＇ confidence in conquering disease and it can make patient actively cooperate with the doctors to complete the operation.
