注射mNGF治疗危重型小儿手足口病的效果及对患儿体液免疫功能和细胞因子的影响

摘　　要：目的:研究注射mNGF治疗危重型小儿手足口病的效果及对患儿体液免疫功能和细胞因子的影响。方法:选取2011年3月-2016年7月在本院接受治疗的危重型小儿手足口病患儿50例作为研究对象,将其按照随机数字表法分为两组,每组25例,对照组给予双黄连常规治疗,观察组在常规治疗的基础上注射mNGF进行治疗,记录两组患儿相关临床数据[包括免疫球蛋白M(IgM)、IgG、IgA及CD4^+、CD8^+、CD4^+/CD8^+水平]并进行比较,对比两组患儿的治疗效果和不良反应发生情况。结果:治疗前,两组IgM、IgG和IgA水平比较,差异均无统计学意义(P>0.05);治疗后,两组IgM、IgG和IgA三项指标均较治疗前显著升高,且观察组比对照组升高更明显(P<0.05);治疗前,两组CD4^+、CD8^+、CD4^+/CD8^+水平比较,差异均无统计学意义(P>0.05);治疗后,两组CD4^+、CD8^+、CD4^+/CD8^+水平均较治疗前显著升高,且观察组比对照组升高更明显(P<0.05);两组总有效率比较,差异有统计学意义(P<0.05);两组患儿治疗期间均未发生肝肾等脏器功能的异常,观察组1例患儿出现注射部位轻微疼痛,停药后症状自行消失。结论:对于危重型小儿手足口病的治疗,注射mNGF能明显改善患儿体液免疫功能和细胞因子的水平,表现出效果显著和不良反应少的特点,可以在临床上进行推广应用。Objective： To explore the effect of mNGF on children with severe hand-foot-mouth disease and its influence on humoral immune function and cytokines.nethod： Fifty patients with severe hand-foot-mouth disease treated in our hospital from March 2011l to July 2016 were selected as the subjects, they were randomly divided into the observation group and the control group, 25 cases in each group.In the control group, patients were treated with Shuanghuanglian, patients in the observation group were treated with mNGF on the routine basis.The levels of immunoglobulin M （IgM）, IgG, IgA, CD4^＋, CD8^＋, CD4^＋/CD8^＋ levels of two groups were compared, the treatment effect and the incidence of adverse reactions of two groups were observed.Result： IgM, IgG and IgA levels of two groups before treatment, there were no significant differences （P〉0.05） .IgM, IgG and IgA levels of two groups after treatment significantly increased, and the observation group increased more than those of the control group （ P〈0.05 ） .CD4^＋, CD8^＋, CD4^＋/CD8^＋ levels of two groups, there were no significant differences （ P〉0.05 ） .CD4^＋, CD8^＋, CD4^＋/CD8^＋ levels of two groups after treatment significantly increased, and the observation group increased more than those of the control group （ P〈0.05 ） .The effect of the treatment group was more effective than that of the control group （ P〈0.05 ） .The incidence of adverse events of two groups was compared, during the treatment children of two groups did not occur liver and kidney and other organ dysfunction, the observation group had 1 case of children with mild pain injection site, symptoms disappear after stopping. Conclusion： Severe hand-foot-mouth disease treated with injection of mNGF can significantly improve the level of humoral immune function and cytokines, has a significant effect and less adverse reactions, can be used in clinical practice.
