经阴道超声诊断陈旧性宫外孕的回顾性分析

摘　　要：目的研究经阴道超声诊断陈旧性宫外孕的临床应用效果。方法回顾性分析我院2015年3月至2016年3月80例经阴道超声诊断与经腹超声诊断的陈旧性宫外孕患者的临床资料,均经手术及病理证实。比较经阴道超声诊断与经腹超声诊断结果的差异,分析经阴道超声诊断的声像图表现。结果以病理诊断结果为准,经阴道超声诊断符合率为87.50%,高于经腹部超声的47.50%,经阴道超声诊断的误诊率为17.95%低于经腹部超声的64.10%,差异均有统计学意义(P<0.05)。经阴道超声声像图显示51例混合性包块,17例实块型包块,6例囊液型包块;59例子宫增大,31例宫内膜增厚,47例盆腔积液。结论经阴道超声对陈旧性宫外孕的诊断价值优于经腹超声,诊断符合率更高,误诊率较低,且声像图有一定特征表现,有利于提高诊断准确率。Objective To study the clinical application effect of transvaginal ultrasound in the diagnosis of old ectopic pregnancy. Methods The clinical data of 80 cases of patients with ectopic pregnancy diagnosed by transvaginal ultrasound and transabdominal ultrasound in our hospital from March 2015 to March 2016 were retrospectively analyzed. All the patients were confirmed by operation and pathology. The results of transvaginal ultrasound and transabdominal ultrasound diagnosis were compared, and the sonographic features of transvaginal ultrasound were analyzed. Results Based on the results of pathological diagnosis, the diagnostic accuracy rate of transvaginal ultrasound was 87.50%, which was higher than 47.50% of transabdominal ultrasound, and the misdiagnosis rate of transvaginal ultrasound was 17.95%, which was lower than 64.10% of transabdominal ultrasound, the differences were statistically significant （P〈0.05）. Transvaginal sonography revealed 51 cases of mixed mass, 17 cases of solid block, 6 cases of cystic fluid mass, 59 cases of uterine enlargement, 31 cases of endometrial thickening, and 47 cases of pelvic effusion. Conclusion The value of transvaginal ultrasound in the diagnosis of old ectopic pregnancy is better than that of transabdominal ultrasound. The diagnostic coincidence rate is higher, and the misdiagnosis rate is lower. There are some characteristics of the sonogram, which is helpful to improve the accuracy of diagnosis.
