摘 要:目的研究授权教育对复杂性肛瘘患者术后康复进程及排便功能的影响。方法选择2015年11月至2016年5月间在本院住院手术治疗的复杂性肛瘘患者67例进行分组研究。随机分为观察组(34例)及对照组(33例),观察组给予授权教育,对照组给予常规健康教育,比较两组入院时、术前、术后1月自我效能量表差异,观察两组术后康复进程及术后1月排便改善情况。结果观察组术前及术后1月自我效能评分(3.23±0.24)分、(3.55±0.38)分均显著高于对照组(2.41±0.52)分、(2.60±0.91)分(P<0.05)。观察组伤口停止渗液、创口肿胀消退、疼痛消失、创口完全愈合的时间分别为(4.13±0.85)d、(4.13±1.67)d、(13.62±2.41)d、(17.13±3.44)d,均少于对照组(均P<0.05)。术后1月观察组排便功能正常达91.18%,无完全失禁病例,优于对照组(P<0.05)。结论授权教育可以更好的提高复杂性肛瘘患者自我效能,从而加快术后康复进程,且有利于术后排便功能恢复,值得推广。Objective To study the effect of empowerment education on rehabilitation and defecation functions of patients with complex anal fistula after surgery. Methods 67 patients with complicated anal fistula who were admitted to our hospital from November2015 to May 2016 were randomly assigned to the treatmentgroup(34 cases) and the control group(33 cases).The observation group was givenempowerment education,and the control group conventional education. The difference of self-efficacy scale uponadmission,preoperative and 1 month postoperative was compared between the two groups. and the rehabilitation and defecation were compared between the two groups 1 month after the operation.Results The scores of self-efficacy preoperative [(3.23 ± 0.24)] and 1 month postoperative[(3.55±0.38)]in the treatment group were significantly higher than those of the control group [(2.41±0.52) and(2.60±0.91),respectively,(P〈0. 05)].The time to disappearance of oozing,swelling of the wound,and pain and the time to complete would healing in the treatment group were(4.13±0.85)d,(4.13±1.67)d,(13.62±2.41)d and(17.13±3.44) d,which were significantly lower than those in the control group(P〈0. 05). The defecation functions was normal in 91.18% of the patients the treatmentgroup and no patient had complete incontinence.The difference compared with control was significant(P〈0. 05). Conclusion Empowerment education can better improve the self-efficacy of patients with complex anal fistula,speed up the postoperative rehabilitation process,and is beneficial for postoperative bowel function recovery. It has good potential for clinical application.
